1、精神疾病的診斷依據(Ch3補充),各國心理衛生專業人員對於心理疾病的診 斷，主要是依據兩套診斷系統： 1.美國精神醫學會主編的精神疾病診斷與統計手冊（Diagnostic and statistical manual of mental disorders, DSM） 2.聯合國世界衛生組織所主編的國際疾病分類手冊（International classification of diseases, ICD）,五軸診斷法,美國精神醫學會在編訂精神疾病診斷與統計手冊時是採用五軸診斷法早期心理疾病的診斷和一般生理疾病的診斷一樣，使用單軸診斷法,五軸診斷法是從五個向度去評估當事人的身心功能，透過有計畫
2、的、綜合多方面的評估，可以幫助治療師擬定治療計畫和預測治療效果,五軸包括：如表1-1 主要診斷 智能與人格障礙 生理疾病 心理社會與環境問題 功能的整體評估,表1-1 DSM-IV-TR五軸診斷法,診斷 Diagnosis,Axis I : Bipolar disorder, manic with Psychotic feature；R/O: schizophrenia Axis II : suspected antisocial personality trait Axis III: defer Axis IV : poor insight and external attribution；
3、Severity Axis V : GAF = 21-30 (current)；GAF = 31-40 (best in past 12m),主要診斷,在第一軸上面，治療師用來呈現當事人心理疾病的主要診斷一個人如果罹患兩種或以上的疾病時，可同時在第一軸上呈現，必要時可註明哪一個是主要診斷，哪一個是次要診斷,屬於第一軸的心理疾病包括下列的診斷類別：,經常發病於嬰兒、兒童與青少年時期的心理疾病 譫妄、失智，以及失憶和其他認知障礙 器質性心理疾病 物質相關障礙 精神分裂症和其他精神病障礙 情緒障礙 焦慮障礙 身心症,解離症 性功能和性別認同障礙 飲食障礙 睡眠障礙 衝動控制障礙 適應障礙 未達心理疾
4、病程度 但是需要治療師協助的心理問題,智能與人格障礙,在第二軸上，治療師用來呈現有關智能和人格障礙的診斷由於智能障礙和人格障礙都是屬於長期性的發展障礙，影響患者各方面的功能，為避免治療師在診斷時忽視它的存在，因此診斷手冊才另外使用第二軸來呈現,屬於第二軸的診斷包括以下的心理疾病：,妄想型人格障礙 疏離型人格障礙 怪異型人格障礙 反社會型人格障礙 邊緣型人格障礙,戲劇型人格障礙 自戀型人格障礙 迴避型人格障礙 依賴型人格障礙 強迫型人格障礙 智能障礙,生理疾病,在第三軸上，治療師可以呈現當事人的一般生理疾病的診斷，特別是那些和心理疾病的了解與治療有關的生理疾病第三軸的設計，主要目的在於提醒治療師
5、，在評估當事人的心理疾病時，也要評估可能的生理疾病，以及兩者的相互影響,可能出現在第三軸上的生理疾病如下：,感染性及寄生蟲疾病 腫瘤 內分泌、營養及代謝性疾病與免疫性疾病 血液及造血器官疾病 神經系統及感覺器官疾病 循環系統疾病 呼吸系統疾病 消化系統疾病,生殖泌尿系統疾病 妊娠、生產及產褥期之合併症 皮膚及皮下組織疾病 肌肉骨骼系統及結締組織疾病 先天形成不良 生產前後開始的某些症狀 症狀、病徵及定義不良的狀況 受傷及中毒,心理社會與環境問題,在第四軸上，治療師可以呈現影響當事人心理疾病診斷、治療和預後的心理社會和環境問題所謂心理社會與環境問題是指： 負面的生活事件 環境的困難或限制 家庭或
6、其他人際壓力 欠缺社會支持或人際資源,屬於第四軸的 心理社會與環境問題包括下列幾類：,與家庭有關的問題 與社會環境有關的問題 教育問題 職業問題 居住問題 經濟問題 就醫問題 司法問題 其他問題,案例：,心理社會問題： 主要支持團體的問題：父親會對母親家暴，之前分居，最近父親搬來一起住 社會環境相關的問題：朋友少，人際關係不佳 教育問題：大學，已被休學 職業問題：餐飲打工(高中) 居住問題：住家中 經濟問題：家人提供 使用健康照顧服務問題：服藥不規則，無病識感，認為躁鬱症是遺傳的不必吃藥 牽涉法律及犯罪的相關問題： 其他心理社會及環境的問題： 病前人格及其他：嬌生慣養、堅持己見，自我中心,功能
7、的整體評估,所謂整體功能是指，當事人在心理、社會與職業上的功能而言，特別是指初診時和結案時的功能整體功能的評估分數從1至100，分數愈低則整體功能愈差，分數愈高則整體功能愈好,功能的整體評估的分數，是以一種過度簡化的方式，來呈現當事人心理症狀嚴重的程度，以及心理社會功能好壞的程度 分數的涵義呈現如表1-2,表1-2 DSM-IV-TR心理功能整體評量表,精神狀態檢查Mental Status Examination 案例：,General appearance:normal unkempt childlike ill at easedisheveled old-looking young-lo
