6、Form must have three basic : JSA表格必须包括以下三个基本内容：1 Job Steps工作步骤 2 Potential Accident or Hazardous Conditions潜在事故和风险状况 3 Safe Work Procedures or Control安全操作规程和控制手段JSA的基本内容Production Of a JSA Involves 5 Basic Steps 实施 JSA包括5个基本步骤：1. Select A Job To Be Analyzed 选择合适的工作进行分析 2. Identify and List All Hazar
7、ds 定义和列出所有的危险类别 3. Separate The Job Into Basic Steps将工作内容分解到它的基本步骤 4. Identify The Potential Accident or Hazardous Conditions 识别潜在的事故和危险状态 5. Devise Safe Work Procedures or Controls 给出安全操作规程和控制方法JSA的基本步骤Selecting A Job To Be Analyzed What Type of Jobs Should Have a JSA ? 选择一项工作去做分析 什么类型的工作需要做JSA？1.
8、Job That have a History of, or Potential for Accidents or Incidents 这项工作有事故历史或存在潜在风险 2. Jobs That Should Always Be Done The Same Way 这项工作应该采用同样的方式 3. 12、teps Describe What is Being Done: 确保清楚描述出每步工作做什么： 1. Identify and list each Job step in sequential Order ( Wording should begin with an “Action Word ie: Remove, Open, Weld, Check etc).根据工作的连续次序，定义和列出每一个工作步骤（措词必须以一个动词开头，例如：移除、打开、焊接、检查等等） 2. Limit the number of Steps to Ten(10) Maximum.限定工作步骤不超过10步 3.
13、 If Job has More Than Ten(10) Steps, Segment it and Produce a JSA forEach Segment.如果一个工作超过10步，对此项工作进行分割，并对每个分割进行JSAJSA的实施（步骤三）Potential Problems:潜在问题：Making the breakdown so detailed that an unnecessarily largenumber of steps results分解过于详细，将工作分解成太多的不必要的步骤 Making the job breakdown so general that bas
14、ic steps are not recorded分解过于简单，将工作分解为太少的几部，不能正确反映工作内容JSA的实施（步骤三）开到路边路边 停好位置 拉下手刹 打开警告闪烁牌 打开车门 下车 走向后备箱 插入钥匙 打开后备箱 拿出千斤顶 拿出备胎 松螺丝关键步骤太多 换轮胎顶起车身 移除松开的螺丝 移开爆胎 放好新轮胎 放下车身 紧固螺丝 将千斤顶和爆胎放回后备箱 锁好后备箱 进入车内和启动车辆 给出信号、加速、上路 关键步骤太少 换轮胎JSA的实施（步骤三） 停车 取下轮胎 上备胎 开走JSA的实施（步骤三） 停好车、拉手刹、放警示牌 从后备箱拿出千斤顶和备胎 松螺丝 顶起车身 移除爆胎
15、，上备胎 放下车身 拧紧螺丝 放好爆胎和千斤顶正确和恰当的分解步骤 换轮胎JSA的实施（步骤四）Identify The Potential Accidents or Hazardous Conditions for Each Job Steps 为每一个工作步骤定义潜在的事故或风险状态In conjunction With The Identified Hazard List from Step 2, Use the following Catch Phrases:通过步骤2定义出的风险清单，使用如下： p 被XXX打击、冲击 p 与XXX接触 p 被XXX接触 p 被夹（在里，在下，在之间） p 暴露于（温度、化学品、噪声等） p 滑到、跌倒、跌落，从高处或同一平面 p 用
