1、上海居家养老服务的现状和对策探讨摘 要上海自 1979年在全国率先进入人口老龄化社会行列以来一直是全国老龄化程度最高的地区为应对上海人口老龄化和高龄化所引起的养老问题上海自 2000年起试行居家养老服务在上海开展居家养老服务的 5年中居家养老社会资源供给的不足和老年人居家养老服务需求之间的矛盾越来越突出相关问题急待解决本文试图通过对上海居家养老服务现状发达国家或地区实践活动的分析研究针对上海的现实情况对上海居家养老服务现状的对策提出作者个人的建议本文第一章首先对居家养老服务的相关概念作了解释第二章着重分析了上海人口老龄化的情况养老福利事业现状老年人养老服务需求和居家养老服务的现状第三章细致研究
2、了几个代表性国家或地区的养老服务实践情况借鉴了国外居家养老服务的社会化模式英国社区照顾模式和澳大利亚家庭老年护理服务以房养老模式美国老年人倒按揭制度专门保险制度模式日本介护保险制度以及我国香港特别行政区的安老服务模式的实践经验和特点本文第四章根据前两章对上海居家养老服务现状分析和发达国家或地区实践经验的借鉴从构建多层次居家养老社区支持体系探索建立护理保险制度探索建立倒按揭制度等三个方面提出了对上海居家养老服务对策的建议关键字社会福利居家养老社区对策THE RESEARCH OF THE ACTUALITY AND COUNTERMEASURES AGAINST THE SERVICES OF
3、PROVIDING FOR THE AGED AT HOME IN SHANGHAIABSTRACTFrom 1979, when taking the lead in getting into the range of thepopulation ageing trend society, Shanghai has been the region of the highestextent of the population-ageing trend in China. In order to answer theproblem aroused by the population ageing
4、 trend, “providing for the aged”, wehas tried out the services of providing for the aged at home in Shanghai since2000. During the five years of carrying out the services, the contradictionbetween the shortage of social resources of providing for the aged at homeand a great demand of the services of
5、 providing for the aged at home hasbecome more and more critical, and the relevant problems are expected to besolved urgently. This article tries to make the personal proposal for theactuality and countermeasures against the services of providing for the agedat home in Shanghai based on the actual s
6、ituation in Shanghai, thoughanalysis and study on the actuality of the services of providing for the aged athome in Shanghai and the practice activities in developed countries orregions.Chapter 1 of this article explains the relevant concepts involved in theservices of providing for the aged at home

Chapter 2 mainly analyses thestatus of the population ageing trend in Shanghai, the actuality of the suppliesfor the aged welfare, the demand of the services of providing for the aged,and the actuality of the services of providing for the aged at home. Chapter 3studies in detail the practice of se
8、veral representative countries or regions.Drawing lessons from the socialization mode of the services of providing forthe agedthe community taking care mode in Britain and the familyagedness nurse services in Australia; from the mode of providing for the agedthrough housesthe converse mortgage loan
9、system in U.S.A.; from themode of the special insurance systemthe tend and protection insurancesystem in Japan; from the practice experience and characteristics of satisfyingthe aged services mode in Hong Kong Special Administrative Region. Basedon the above two chapters, which analyze the status of
10、 the services ofproviding for the aged at home in Shanghai and draw lessons from thepractice experience in developed countries or regions, Chapter 4 makesproposal to the countermeasures of the services of providing for the aged athome in Shanghai from the aspects of constructing multi-level communit
11、ysupporting system of providing for the aged at home, searching forestablishing nurse insurance system, and searching for establishing conversemortgage loan system, etc.KEY WORDS: social welfare, providing for the aged at home, community,countermeasure1前言世界人口日趋老龄化问题现在已经是一个全球性问题由于人口加速老龄化趋势的日益严重老年人口的病
12、残率上升社会养老和医疗的压力迅速加剧随着劳动年龄人口减少养老保险和医疗保险基金储备出现不足支付越来越多的老年人口的社会保险基金的风险加大养老问题发展战略正在纳入各国社会和经济发展战略之中摆在各级政府工作的主要议程之上上海自 1979 年在全国率先进入人口老龄化社会行列以来一直是全国老龄化程度最高的地区近几年为应对上海人口老龄化和高龄化所引起的养老问题上海在继续稳步推进机构养老的同时大力探索以居家养老为主要形式的多元化社会化的养老模式并取得了一定的成绩2000 年至今上海接受居家养老服务的老人已达 14 万人次政府为困难和特殊贡献老人提供的服务补贴已达 2000 多万元但随着经济的全球化市场经济
13、竞争激烈资本流动能力增强往往资本流向与经济增长就业增加相关的行业或事业而居家养老社会资源供给的不足和老年人居家养老服务需求之间的供需矛盾在竞争中也越来越突出上海居家养老服务的覆盖面资金来源资源配置政策扶持模式创新等问题已不只是分管老龄工作的民政部门一家的事而逐渐成为上海城市公共管理的一个重要问题世界上许多发达国家或地区已在老年人社会保障和养老服务方面进行了大量的探索和实践本文细致研究了几个代表性国家或地区的养老服务实践情况借鉴了国外居家养老服务的社会化模式英国社区照顾和澳大利亚家庭老年护理服务模式以房养老模式美国老年人倒按揭制度专门保险制度模式日本介护保险制度以及我国香港特别行政区的安老服务模

16、 0 0 5 年 1 1 月 1 8 日上海交通大学 MPA 学位论文 上海居家养老服务的现状和对策探讨1第一章 理论与概念1 . 1 居家养老服务的相关概念1 . 1 . 1 社会保障社会保障Social Security也称社会安全社会保障一词最初是在美国 1935 年的社 会保障法中提出的是特指养老保险制度后来在 1941 年的大西洋宪法中也使用这一概 念直到今天在美国社会保障制度的概念仍然主要是指养老保险还包括遗属和残障保护 后来国际劳工组织在其一系列的公约建议书等文献中引用了此概念但内涵己发生变化是 指国家为公民提供一系列基本生活保障使公民在年老疾病失业灾害及丧失劳动能力等 情况下从国家和社会获得现金和实物帮助的制度我国 50 年代初内务部最早在民政工作中曾使用过社会保障一词1986 年中共中央 关于制定国民经济和社会发展第七个五年计划的建议中明确把社会保障界定为四项内容 即社会保险社会救济社会福利和优抚工作19
