12、工作制度、操作规程、维修程序与流程管理等文件，确保医学装备与支持性设施能得到有效地管理。严格医疗仪器设备、器械、耗材工作和技术管理，强化应用质量，预防和控制应用风险，保障医疗安全本评估标准主要从以下方面开展评估：（100分+56.4分附加分）（一）有适宜的医疗仪器设备管理保障组织、规章制度与人员岗位职责，并健全落实保证机制（15分）；（二）建立健全医疗设备、耗材的论证、招标、采购、验收等制度（ 25分）；（三）日常仓储及使用管理（15分）；（四）有保障设备处于完好状态的制度与规范，急救生命支持系统仪器设备保持完好和待用状态，建立全院应急调配机制（10分）；（五）建立医疗仪器设备保养、维修的管理
13、制度，开展医疗器械临床使用安全控制与风险管理工作，保障医疗安全（15分）；（六）建立医疗器械临床使用安全事件监测与报告制度，加强不良事件管理（5分）；（七）装备引进要与医院功能、技术水平相适应，并符合国家相关规定，应有成本效益、临床使用效果、质量等医疗设备的应用分析制度（5分）；（八）按照大型医用设备配置与使用管理办法，合理配置和使用甲、乙类大型医疗设备（10分）；附件分评估内容（九）建立医疗器械临床试验管理制度、管理流程，试验负责人具有相关技术专业职称（附加分4.2分）；（十）有医用氧舱安全应用管理制度与措施，确保设备安全（附加分18.2分）；（十一）按照法律、法规的规定，使用和管理医用含放

16、记录等，走访.现场了解情况。1. 未设立院级医疗器械临床使用安全委员会，扣3分；有设立，但组成人员缺医疗行政管理、临床医学及护理、医院感染管理、医疗器械保障管理之一者，扣0.5分；2. 设立院级医疗设备管理委员会，但无相应工作流程和规范或有但不健全或未落实，酌情扣0.51.5分。（二）采购管理（25分）建立医疗设备和耗材的论证、招标管理制度,设置了医疗设备采购专家库，有年度采购计划，严格按规定采购设备。71. 查相关制度、专家库名单、采购计划等；2. 从当年所购设备清单中抽取2种设备（其中一台为50万元以上设备）,查其论证、招标、公示、采购合同、验收档案。1. 集中采购、部门采购、自行采购制度缺一项扣0.分；2. 未建立院级医疗设备采购专家库的,扣1.5分；3. 抽查的设备未建立采购档案的,扣3分,有但缺项者,酌情扣0.5-2
