1、注意缺陷多动障碍多动与冲动的定量研究指导教师姓名 秦岭 专业名称 神经病学申请学位类型 专业型硕士摘 要研究目的:通过三轴加速度传感器的测量,对注意缺陷多动障碍(attention difict/hyperactivity disorder ADHD)患儿的多动/冲动行为进行定量研究,并探索实验室环境下能有效筛查 ADHD 多动/ 冲动行为的有效方法。研究方法:实验组由有经验的医师诊断出 ADHD-HI(15 例)及ADHD-C(55 例)患儿,共 70 例(64 男, 6 女) ,对照组为相应年龄、性别的南宁市某小学学生,共 70 例(64 男,6 女) 。采用分析性研究,对各研究对象进行试
2、听整合连续性行为测试(IVA-CPT) ,采集综合尺度控制商数,在测试过程中在研究对象的非主利手腕及脚踝佩戴三轴加速度传感器,采集其记录的 counts 值及能量消耗等数据进行分析研究。研究结果:三轴加速度传感器能够提供描述多动/冲动行为的指标,试验组与对照组手腕及脚踝 counts 均值和能量消耗的 P 值均0.05。三轴加速度传感器及综合尺度控制商数进行平行联合诊断,ROC 曲线下面积为 0.910,对多动/冲动行为的诊断灵敏度为91.4%,假阳性为 21.4%。结论:(1)实验室条件下对 ADHD 多动 /冲动的定量评估必须在限制性场景下进行;(2)加速度传感器能够对 ADHD 多动/冲
3、动行为提供定量的评估;(3)加速度传感器联合 IVA-CPT 进行平行联合诊断多动/冲动有价值,值得在临床诊断试验中推广;(4) 本研究中 90-134 月之间的 ADHD 的多动/冲动行为与年龄的关系不能确定。THE QUANTITATIVE STUDY OF HYPERACTIVITY/IMPULSIVITY SYMPTOM IN ATTENTION-DEFICIT/HYPERACTIVITY DISORDERABSTRACTObjectives: We aimed to quantify the ability of motion sensor data, taken from 3-ax

5、 Cross-section research and Receiver operating characteristic analysis were used on 3-axis acceleration sensor data collected during a limited scene, which cost about 20 minutes, in a sample of 70 ADHD combined-type, diagnosed by experienced clinical doctors, and70control siblings, matched by age, g
6、ender and weight.Results: Two measures of 3-axis acceleration sensor data-mean counts and energy expenditure-yielded a P0.05, indicating an ability to distinguish between cases and controls. The diagnostic efficiency is excellent of the diagnostic way combined with 3-axis acceleration sensor and int
7、egrated visual and auditory continuous performance test (IVA-CPT), yield an area under the curve is 0.910, and the sensitivity up to 90%, and the specificity up to 80%.Conclusions: a. The research about to quantify the hyperactive/impulsive symptom with ADHD must be in a limited scene. b. The measur
8、es of 3-axis acceleration can quantify the hyperactive/impulsive symptom of ADHD. c. The diagnostic efficiency is excellent of the diagnostic way combined with 3-axis acceleration sensor and IVA-CPT. d.
