1、认 证 合 同 书Certification Contract 合同编： Contract Number：一、 总那么 一、General Provisions 以下简称甲方 委托上海英格尔认证以下简称乙方为甲方提供 认证效劳。甲乙双方按照?合同法?等法律法规、行业主部门的规定经平等协商本着“客观、公正、诚信的原那么在理解和同意以下条款的根底上签订本合同并承诺共同遵守。 (The undersigned referred to as Party A), missioned Shanghai Ingeer Certification Ltd (The undersigned referred t
2、o as Party B) to offer Certification for Party A. Both Party A and Party B in accordance with The Peoples Republic of ina Contract Law and other laws and regulations、the provisions of the industry department and after an equal footing in accordance with the spirit of objectivity、 impartiality、indepe
3、ndence、good faith principle, understand and agree to the following terms to sign this contract and promised to abide by.二、 甲方权利和义务二、The rights and Obligations of Party A 甲方有权宣传获准认证的享有由于获准认证以及使用认证标志获得的经济利益。1. Party A has the rights to publicize that certification is permissible to enjoy as authentica
4、tion and the use of certification marks to obtain economic interests. 甲方假设与乙方在认证过程中产生的争议甲方有权向乙方提出申投诉乙方应按照申投诉程序解决争议。2. If Party A and Party B have disputes in the certification process, Party A is entitled to raise plaints to Party B of whi Party B shall then settle the disputes in accordance with th
5、e plaint procedure. 甲方应根据认证程序积极配合乙方的审核向乙方提供充分的信息和记录。否那么应承担信息不准确或不所造成的一切后果。3. Party A, in accordance with certification program shall actively cooperate with the audit of Party B to provide real and adequate information and records for Party B. Otherwise, all the consequences caused by inaccurate or
6、false information shall be accepted. 甲方向乙方提出认证申请时应明确告知乙方前六个月内是否向其他认证机构提交过同样的认证申请但未被授理或审核未通过、已经获得其他机构的认证证书但目前被暂停、撤销、注销等信息。4. When Party A applies for certification to Party B, Party A should inform Party B if the following occur within the last six months: 1.whether or not Part A has submitted an app
7、lication to some other certification bodies or it fails to pass certification. 2. Party A has already acquired the certificate from other certification bodies but the certificate is suspended, terminated or cancelled.5.甲方获证后应确保认证所覆盖的理体系持续有效运行并按承受乙方的监视审核及再认证审核以便乙方验证认证所覆盖的理体系是否持续符合认证要求5. Party A, afte
8、r certified, should ensure that the certification of management systems running shall continue to be enforce and accept the surveillance and certification audit so Party B can verify the certification of management systems has been persistently enforced in pliance with the certification requirements
9、. l 初评获证后在证书有效内须按承受三次监视审核第一次监视审核在初评审核后第6个月进展以后每次监视审核间隔9-12个月。l Preliminary assesent of certification at the certificate validity period has three surveillance assesents; for the first time surveillance assesents in evaluation review is 6 months, ea subsequent surveillance assesents interval is 9-12
10、months. l 再认证获证后在证书有效内须按承受两次监视审核第一次监视审核在再认证审核后0个月进展以后每次监视审核间隔9-12个月。l After re-certification, the certificate is abstained .The second servisory review is seduled within the certificates validity period. Servision and examination for the first time after the first 10 months for re-certification audi
11、t and servision and examination is to be conducted after the time intervals of 9-12 months.l 再认证应在证书到前三个月施行。l Re-certification should be implemented three months before the certificate expires 甲方应按照双方约定的时间和数额向乙方交纳相关认证费。 6. Party A in accordance with the timing and amount agreed on by the two sides t
12、o the Party shall pay fees related to accreditation. 甲方获证后在证书有效内如发生重大顾客投诉、重大质量、消费、环境、平安事故或发生以下重大变化应及时书面乙方。7. After Party A abstain the certificate within the validity period of the certificate, major customer plaints, significant quality, production, environment, safety incidents, or during the foll
13、owing major anges should be promptly notified to Party Bl 法律地位、经营状况、组织状态或所有权；l legal status, operating conditions, organizational status or ownership l 组织或理层如关键的理、决策、或技术人员；l organization or management (su as key management, decision-making, or tenical staff)l 和场所；l contact addresses and placesl 获证理体
14、系覆盖的认证范围；l certified management system covering the scope of certificationl 理和过程的重大变更；l major anges of management and process 乙方暂停或撤消证书时甲方应立即停顿使用认证证书、认证标志及其复印件。When Party B decides to suspend or revoke the certificate, Party A should immediately stop using the certificate, the certification logo and
15、 the copy of the certificate.甲方获证后应根据?认证认可标徽标志及证书的使用规那么?正确使用认证证书和认证标志。After Party A obtains certificate, authentication certificates and certification mark, it should be properly use under the Certification and Accreditation standard logo signs and rules regarding use of the certificate.8. 甲方如注销证书应书面乙方证书注销手续并将证书交还乙方。8. If Party A decides to cancel the certificate, Party A should notify Party B and Party B will handle the cancellation procedures and the certifi
