12、号： ；资质证书编号： ；发证机关： ； 安全生产许可证有效期： ；安全资格证书编号：三、工程承包范围承包范围为 工程设计施工图包含的人工挖孔桩的全部施工内容。成孔（含扩底）、施工过程中的孤石及不利地质条件的处理、护壁模板安拆、护壁钢筋制安、砼护壁浇筑、孔底清渣、桩孔排水、施工场地排水沟设置、砌井圈、安全设施及安全防护措施等人工挖孔桩施工全过程的所有人工费及现场安全文明施工费。本合同承包范围还包括所有施工用机械、机具、工具及辅助材料，按重庆市要求孔内垂直运输必须使用电动卷扬机。余土就地回填（如确有余土放不下由甲方负责运走），不允许余土倒在基础梁范围内。四、工程承包方式和价格1 人工挖孔桩的所有
13、人工费、机械费、临时设施费、钢筋安装费、砼浇筑费、保险费、所有辅助材料消耗品费用及第二条工程承包范围内，不论桩径大小，桩身深度和发生何种情况，承包单价均不得调整。护壁钢筋、护壁混凝土材料由甲方供应，井口架子由乙方负责搭拆，钢管、竹跳（或木板）、彩条布板由甲方提供。2 承包单价：按设计图纸、设计变更、施工合同中工程承包范围组织施工，双方议定自甲方移交的场平标高以下土方、页岩、小于桩井口半径的孤石(不论孤石在桩外有多大，均只按桩井口内的大小计算，不论用什么机械施工)均按 元/m3；大于井口半径的孤石单价为 元/m3;嵌入岩层的石方，必须使用空压机风镐的 元/m3 ;使用水钻施工的按 元/m3。含本
14、合同包含所有工作内容以及机械进出场费、孔内照明费、送风费、砌井圈、安全文明施工费、现场管理费和利润、税金等一切费用。3 由乙方在甲方指定地点安装施工用水、电，水、电费用由甲方承担，如发现浪费现象甲方有权做罚款处理。五、工程合同工期1 本工程工期为 日历天，开工日期为 年 月 日，完成时间 年 月 日，不良天气下雨等影响、法定节假日等因素均包括在合同总工期内。2 由于连续停电、停水超过24小时或不可抗拒因素引起的工期延误，其工期签证顺延。3 由甲方通知和设计变更引起乙方停工1天以上，工期相应顺延。4 工期奖罚：不能按合同工期完成，每拖延一天罚款2000元，每提前一天奖励1000元。六、工程质量1
15、 按照现行国家桩基质量验收规范及工程质量评定标准进行验收，工程质量达到合格标准，其他符合桩基检测技术规范要求。2在施工过程中防止桩孔倾斜及桩顶位移偏差大，应严格按图定位，并有复检。挖孔过程中，应随时用线坠吊放中心线，发现偏差过大立即纠偏。要求每次支护壁模板都要吊线一次（以顶部中心的十字线为准）。扩底时，应从孔中心点吊线放扩底中心桩。成孔完毕后，应立即检查验收，避免晾孔时间过长，造成不必要的塌孔。七、工程安全1乙方在施工过程中必须严格按照安全生产规范、操作规程组织施工，制订切实可行的安全措施并执行。要求有可靠合理的安全应急预案。2 每次开工前必须检测井下的有毒有害气体，并应有足够的安全防护措施，有专门向井下送风的设备；孔口四周必须设
