1、 afp实行“零”病例报告制度篇一：卫生部规定报告的14类AFP病例 卫生部规定报告的14类急性弛缓性麻痹（AFP）病例 所有15岁以下出现急性弛缓性症状的病例，和任何年龄临床诊断为脊灰的病例均作为AFP病例。 AFP病例的诊断要点：急性起病、肌张力减弱、肌力下降、腱反射减弱或消失。 常见的AFP病例包括以下疾病： 1. 脊髓灰质炎 2. 格林巴利综合征（感染性多发性神经根神经炎，GBS） 3. 横贯性脊髓炎、脊髓炎、脑脊髓炎、急性神经根脊髓炎 4. 多神经病（药物性多神经病，有毒物质引起(来自:WwW.CssY 书业 网:afp实行“零”病例报告制度)的多神经病，原因不明性多神经 病） 5.

3、P主动监测报告制度 急性弛缓性麻痹（AFP）病例主动监测报告制度 脊髓灰质炎（下称脊灰）是严重危害儿童健康的急性肠道传染病。2003年，我国积极响应世界卫生组织（WHO）提出的消灭脊灰目标，在20032010年全国保持无脊髓灰质炎状态行动计划中明确规定发生脊灰野病毒输入和脊灰疫苗衍生病毒循环的疫情，即为突发公共卫生事件。根据北京市急性弛缓性麻痹（AFP）病例监测方案，结合我院实际情况，特对急性弛缓性麻痹（AFP）病例主动监测报告制度进行修改和完善。 1、监测目的：及时发现急性弛缓性麻痹（AFP）病例，避免迟报、漏报现象。 2、监测病例定义： 2.1急性弛缓性麻痹（AFP）病例 任何小于15岁出
4、现急性弛缓性麻痹（AFP）症状的病例，和任何年龄临床诊断为脊髓灰质炎的病例均作为AFP病例。 AFP病例不是一个单一的疾病种类，而是以急性起病、肌张力减弱、肌力下降和腱反射减弱或消失为主要特征的一组症候群。常见的AFP病例包括以下疾病： （1）脊髓灰质炎； （2）格林巴利综合征（感染性多发性神经根神经炎，GBS）； （3）横贯性脊髓炎、脊髓炎、脑脊髓炎、急性神经根脊髓炎； （4）多神经病（药物性多神经病，有毒物质引起的多神经病、原因不明性多神经病）； （5）神经根炎； （6）外伤性神经炎（包括臀肌药物注射后引发的神经炎）； （7）单神经炎； （8）神经丛炎； （9）周期性麻痹（包括低钾性麻痹、
5、高钾性麻痹、正常钾性麻痹）； （10）肌病（包括全身型重症肌无力、中毒性、原因不明性肌病）； （11）急性多发性肌炎； （12）肉毒中毒； （13）四肢瘫、截瘫和单瘫（原因不明）； （14）短暂性肢体麻痹。 2.2高危AFP病例 凡满足下列条件之一，定为高危AFP病例：（1）年龄小于5岁、接种口服脊髓灰质炎减毒活疫苗（OPV）次数少于3次或服苗史不详、未采或未采集到合格粪便标本的AFP病例；（2）临床高度怀疑为脊灰的病例（年龄小于5岁，发热3天后出现弛缓性麻痹，麻痹时伴有发热，退热后，出现躯体或四肢肌张力减弱、深部腱反射减弱或消失，并出现不对称性或双侧弛缓性麻痹，重症伴有呼吸肌麻痹，无感觉障碍
6、，后期肌肉萎缩，临床上不能排除脊灰的AFP病例）。 2.3 聚集性临床符合病例 同一乡（镇、街道）或相邻乡（镇、街道）发现2例或2例以上的临床符合病例，发病时间间隔2个月以内。 2.4 脊灰疫苗衍生病毒（VDPV）循环病例（cVCPVs） 如发生2例或2例以上相关的VDPV病例，则视为VDPV循环（cVDPVs）。 3、监测要求：包括主动监测和主动搜索。 3.1主动监测内容：医院指定专人负责，每旬开展1次 3.1.1设置主动监测科室：所有易发现AFP病例的科室定为主动监测科室，一般为：儿科、神经内科、内科、传染科、急诊科、针灸科、骨科等 3.1.2疫情管理人员：监测人员主动到监测科室，查询AF
7、P病例，填写“医院AFP主动监测旬访表”，发现漏报AFP病例，按要求开展调查和报告。 3.2 主动搜索内容：北京市疾控中心在每年6-7月对全市二级及以上医院，包括部队和厂矿医院完成上一年度病例的主动搜索检查，搜索对象为卫生部规定的14种15岁以下AFP病例，包括医院住院病例和门诊就诊病例。 4、AFP病例报告（疫情报告） 4.1 AFP病例快速报告 临床科室发现AFP病例后，马上报告医院疫情办，同时填写传染病疫情卡片进行网络直报，在网上报告卡的疾病名称中选择其他疾病，在备注中填写AFP，其他项目按原疫情报告规定填写。疫情办在12小时内电话报至朝阳区疾控中心。 4.2 AFP病例旬报 疫情办指定
