1、 辛伐他汀滴丸说明书(共7篇篇一：辛伐他汀滴丸 辛伐他汀滴丸 【药品名称】 通用名称：辛伐他汀滴丸 英文名称：Simvastatin Pills 【适应症】 1.高脂血症（1）对于原发性高胆固醇血症、杂合子家族性高胆固醇血症或混合性高胆固醇血症的患者，当饮食控制及其他非药物治疗不理想时，辛伐他汀可用于降低升高的总胆固醇、低密度脂蛋白胆固醇、载脂蛋白B和甘油三脂。且辛伐他汀升高高密度脂蛋白胆固醇，从而降低低密度脂蛋白/高密度脂蛋白和总胆固醇/高密度脂蛋白的比率。（2）对于纯合子家族性高胆固醇血症患者，当饮食控制及非饮食疗法不理想时，辛伐他汀可用于降低升高的总胆固醇、低密度脂蛋白胆固醇和载脂蛋白B

5、品名： 英文名：Simvastatin Tablets 汉语拼音：Xinftdin Pin 本品主要成份为：辛伐他汀。 结构式：（参见辛伐他汀胶囊） 分子式：C25H38O5 分子量：418.57 【性状】 薄膜衣片，除去薄膜衣后显白色或类白色。 【药理毒理】 本品本身无活性，口服吸收后的水解产物在体内竞争性地抑制胆固醇合成过程中的限速酶羟甲戊二酰辅酶A还原酶，使胆固醇的合成减少，也使低密度脂蛋白受体合成增加，主要作用部位在肝脏，结果使血胆固醇和低密度脂蛋白胆固醇水平降低，中度降低血清甘油三酯水平和增高血高密度脂蛋白水平。由此对动脉粥样硬化和冠心病的防治产生作用。 在小鼠，给34倍人用剂量可以
6、致癌，但在人类大规模长期临床试验中未见肿瘤发生增加。已有的研究未发现本品有致突变作用。 【药代动力学】 本品口服吸收良好，吸收后肝内的浓度高于其他组织，在肝内经广泛首关代谢，水解为代谢产物，以-羟酸为主的三种代谢产物有活性。本品及-羟酸代谢物的蛋白结合率高达95%，达峰时间为1.32.4小时，T1/2为3小时。60%从粪便排出，13%从尿排出。治疗2周可见疗效，46周达高峰，长期治疗后停药，作用持续46周。 【适应症】 1高脂血症 （1）对于原发性高胆固醇血症、杂合子家族性高胆固醇血症或混合性高胆固醇血症的患者，当饮食控制及其他非药物治疗不理想时，辛伐他汀可用于降低升高的总胆固醇、低密度脂蛋白

8、一般始服剂量为每天10mg，晚间顿服。对于胆固醇水平轻至中度升高的患者，始服剂量为每天5mg。若需调整剂量则应间隔四周以上，最大剂量为每天40mg，晚间顿服。当低密度脂蛋白胆固醇水平降至75mg/dL （1.94mmol/L）或总胆固醇水平降至140mg/dL（3.6mmol/L）以下时，应减低辛伐他汀的服用剂量。 （2）纯合子家族性高胆固醇血症：根据对照临床研究结果，对纯合子家族性高胆固醇血症病人，建议辛伐他汀40mg/d晚间顿服，或80mg/d分早晨20mg、午间20mg和晚间40mg三次服用。辛伐他汀应与其他降脂疗法联合应用（如低密度脂蛋白提取法），当无法使用这些方法时，也可单独应用辛伐
9、他汀。 （3）冠心病：冠心病患者可以每天晚上服用20mg作为起始剂量，如需要剂量调整，可参考以上说明(高胆固醇血症用法与用量)。 （4）协同治疗：辛伐他汀单独应用或与胆酸螯合剂协同应用时均有效。对于已同时服用免疫抑制剂类药物的患者，辛伐他汀的推荐剂量为每天10mg。 （5）肾功能不全：由于辛伐他汀由肾脏排泄不明显，故中度肾功能不全病人不必调整剂量；对于严重肾功能不全的患者（肌酐清除率小于30ml/min），如使用剂量超过每天10mg时应慎重考虑，并小心使用。 【不良反应】 辛伐他汀一般耐受性良好，大部分不良反应轻微且为一过性。在临床对照试验中只有少于2%的病人因辛伐他汀的不良反应而中途停药。在

11、红、呼吸困难以及不适。 实验室检查发现：血清氨基转移酶显著和持续性升高的情况罕有报导。肝功能检查异常为轻微或一过性。来源于骨骼肌部分的血清磷酸肌酸激酶（CK）升高的情况也有报告。 【禁忌】 （1）对任何成分过敏者。 （2）活动性肝炎或无法解释的持续血清氨基转移酶升高者。 （3）与四氢萘酚类钙通道阻滞剂米贝地尔合用。 【注意事项】 （1）病人接受辛伐他汀治疗以前应接受标准胆固醇饮食并在治疗过程中继续使用。 （2）肝脏反应。本药应慎用在大量饮酒和/或有肝病历史的病人。有活动性肝病或无法解释的氨基转移酶升高者应禁用辛伐他汀。在临床实验中，有少数服用辛伐他汀的患者有显著的血清氨基转移酶持续升高（超过正
12、常值3倍以上）的现象。但停药后，则氨基转移酶可回复至治疗前水平，但无黄疸或其他有关的临床症状或体征，亦无过敏现象。建议在治疗前对于氨基转移酶有升高现象的患者应加强检查并多加留意。如果病人的氨基转移酶有继续升高的表现，特别是氨基转移酶升高超过正常值3倍以上并保持持续，则应予停药。与其他降脂药相同，应用辛伐他汀治疗的患者氨基转移酶中等程度升高（低于正常值3倍的情况）亦有报道。这些变化通常在应用辛伐他汀治疗后不久即有出现，但一般为一过性且不伴随任何症状，所以不必停药。 （3）肌肉反应。应用辛伐他汀治疗的患者普遍有肌酸激酶（CK，来自骨骼肌）轻微的一过性升高，但这些并无任何临床意义。对于有弥漫性的肌痛
13、、肌软弱或/和显著的肌酸激酶（CK）升高（大于正常值十倍以上）的情况应考虑为肌病，因此应要求病人若发现有不可解释的上述肌病征象应立即告诉医生。若发现肌酸激酶（CK）显著上升或诊断或怀疑肌痛，应立即停止辛伐他汀的治疗。对于有急性或严重的条件暗示的肌病及有因横纹肌溶解而导致二次急性肾衰竭倾向的病人应停止甲基羟戊二酰辅酶A（HMG-CoA）还原酶抑制剂的治疗。 （4）眼科检查。即使在没有任何药物治疗时，随着年龄增长晶状体混浊的发病率亦会增加，长期临床研究资料显示，辛伐他汀对人体晶状体无不良作用。 （5）纯合子型家庭性高胆固醇血症。由于纯合子型家族性高胆固醇血症的患者低密度脂蛋白（LDL）受体的完全缺
14、乏的缘故，辛伐他汀对此类病人的治疗效果不大理想。 （6）高三酰甘油血症。辛伐他汀只有中等程度降低三酰甘油（甘油三酯）的效果，而不适合治疗以三酰甘油升高为主的异常情况（如、及型高脂血症）。 （7）对酒精饮用量过大和/或有既往肝脏病史的患者，应谨慎使用本品。 【孕妇及哺乳期妇女用药】 （1）尚无孕妇用辛伐他汀的资料。妊娠期妇女禁用辛伐他汀。因为动脉粥样硬化是慢性过程，所以妊娠期停用降脂药对治疗原发性高胆固醇血症的远期效果影响甚少。而且胆固醇及其生物合成途径的其他产物是胎儿发育的必需成份，包括类固醇和细胞膜的合成。因为甲基羟戊二酰辅酶A（HMG-CoA）还原酶抑制剂如辛伐他汀能降低胆固醇的合成，而且
15、也可以降低胆固醇生物合成通路的其他产物。所以孕妇服用辛伐他汀可能有损于胎儿。在育龄妇女中，辛伐他汀只能用于那些怀孕可能性很小的妇女。若妇女在服药过程中怀孕，则应停用辛伐他汀并告知对胎儿可能造成的损害。 （2）目前还不了解辛伐他汀及其代谢产物是否经人乳分泌，因为许多药物经人乳分泌且有潜在的严重副作用，故服用辛伐他汀的妇女不宜给予母乳。 【儿童用药】 儿童用药的安全性和有效性尚未确定。辛伐他汀目前不推荐给儿童服用。 【老年患者用药】 在老年患者（大于65岁），应用辛伐他汀的对照临床试验中，其对于降低总胆固醇和低密度脂蛋白(LDL)胆固醇的效果与其他人群的结果相同，而不良反应和试验室检查异常的出现频
16、率亦无明显增多。 【药物相互作用】 （1）当辛伐他汀与其他在治疗剂量下对细胞色素P4503A4有明显抑制作用的药物（如：环孢霉素、米贝地尔、伊曲康唑、酮康唑、红霉素、克拉霉素和奈法唑酮）或纤维酸类衍生物或烟酸合用时，导致横纹肌溶解的危险性增高。 （2）本品与甲基羟戊二酰辅酶A（HMG-COA）还原酶抑制剂合并用药会增加肌病的发生率和严重程度，这些药物包括吉非贝齐和其他贝特类，以及降脂剂量的烟酸（大于等于1g/d）。此外，血浆中高水平的甲基羟戊二酰辅酶A（HMG-COA）还原酶抑制剂的活性增高也会增加肌病的危险。辛伐他汀和其他甲基羟戊二酰辅酶A（HMG-COA）还原酶抑制剂由细胞色素P450的同
17、功酶3A4所代谢。数种在治疗剂量对此代谢途径有明显抑制作用的药物能增高甲基羟戊二酰辅酶A （HMG-COA）还原酶抑制剂的血药水平，并因而增加肌病的危险。这些药物包括环孢菌素、四氢萘酚类、钙通道阻滞剂米贝地尔、伊曲康唑、酮康唑及其他抗真菌唑类、大环内酯类抗生素红霉素和克拉霉素，以及抗抑郁药奈法唑酮。 （3）香豆类衍生物：临床研究曾发现辛伐他汀能中度提高香豆类抗凝剂的抗凝效果。故成人早期应用抗凝血治疗及并用辛伐他汀时应多次检查凝血酶原时间，籍此确定凝血酶原时间没有显著改变。当服用香豆类衍生物的病人，已有一个稳定的凝血酶原时间后，仍推荐在固定的期间内继续作凝血酶原时间的监察。如果辛伐他汀的剂量有变
18、动，应同样执行以上的程序。在未服用抗凝血剂的病人中，辛伐他汀治疗从未有报导对出血或凝血酶原时间有影响。 【药物过量】 【规格】 （1）5mg （2）10mg （3）20mg篇三：辛伐他汀说明书 【辛伐他汀片药品名称】 辛伐他汀片 【辛伐他汀片成分】 辛伐他汀片主要成分为辛伐他汀。 【辛伐他汀片性状】 辛伐他汀片为白色或类白色片。 【辛伐他汀片药理作用】 辛伐他汀片为甲基羟戊二酰辅酶A（HMGCoA）还原酶抑制剂，抑制内源性胆固醇的合成，为血脂调节剂。 【辛伐他汀片药代动力学】 辛伐他汀口服后对肝脏有高度的选择性，其在肝脏中的浓度明显高于其它非靶性组织，辛伐他汀的大部分经肝组织吸收，主要作用在肝
19、脏发挥，随后从胆汁中排泄。只有低于5%口服剂量的辛伐他汀活性结构在外围中发现，而其中95%可与血浆蛋白结合。 【辛伐他汀片毒理研究】 文献资料表明，辛伐他汀片有降低高脂血症家兔血清、肝脏、主动脉中胆固醇（TC）的含量，降低极低密度脂蛋白胆固醇（VLDLC）、低密度脂蛋白胆固醇（LDLC）水平的作用。 【辛伐他汀片适应症】 高胆固醇血症：饮食疗法及其他非药物治疗效果欠佳时，可应用辛伐他汀降低原发性高胆固醇血症患者的总胆固醇和低密度脂蛋白胆固醇。辛伐他汀同时可升高高密度脂蛋白胆固醇并因此降低低密度脂蛋白胆固醇、及总胆固醇/高密度脂蛋白胆固醇的比率。 在合并患高胆固醇血症和高甘油三酯血症的病人，且高
