2、ply of air.104Entry and Work inside Confined Space (2) Safety Measures:q Permit to Work obtained and approved by AMD EHS Section.q Safety certification of confined space by Confined Space Safety Assessor prior to and during entry and work inside confined space. q Periodic gas testing.q Provision of

3、continuous forced ventilation.q Provision of adequate and suitable lighting.q Portable lighting operated at a voltage not exceeding 55 volts (AC) between the conductor and earth or 110 volts (DC).qBuddy System.q Attendant stationed outside the confined space.q Use of PPE such as safety belts, harnes

4、s, SCBA etc.q Personnel working inside shall wear personal gas monitoring device.q Energy Control Procedures.105Entry and Work inside Confined Space (3) Contractor Works:q As stated in Factories (Safety Training Courses) Order, any person engaged to carry out the work inside the confined space shall

5、 have completed the Safety Orientation Course (Manhole). q As stated in Factories (Safety Training Courses) Order, any person engaged to supervise the work carried out inside the confined space shall have completed the Safety Instruction Course (Manhole) conducted by Ministry of Manpower, Singapore.

6、q All contractors shall also attend “Confined Space Safety Training” organized by AMD EHS Section.q Contractor company engaged in confined space work shall employ or engage a confined space safety assessor for the safety certification of confined space. q Copies of testimonials shall be submitted to

7、 AMD EHS Section for verification.106Use of Mechanical Material Handling Equipment (1) Requirements:q All statutory requirements are complied with.q Regular inspection and maintenance of all mechanical material handling equipment.q Statutory testing / examination by Professional Engineer (PE) for cr

8、anes, gondola and other lifting machines / lifting appliances / lifting gears.q Safe Work Procedure to be kept available.q Inspection of PPE such as safety harness etc.q Permit to Work obtained prior to use at AMD premises.q Barricading of areas where lifting machines are operated to prevent unautho

11、sPlease refer to attached link for list of Training Centers approved by Chief Inspector of Factories:http:/www.mom.gov.sg/MOM/OSD/Others/ATPList.pdf 110Use of Mechanical Material Handling Equipment (5) Permit to Work System:qPrior to use of any lifting machines or forklift trucks in AMD premises, pe

12、rmit to work shall be raised and approval shall be obtained from AMD EHS Section.q For gondola, permit shall be approved by EHS prior to erection, installation, alteration, repositioning or dismantling.q The starting and ending date shall be clearly stated in the permit. q If there is any change in

13、work duration, driver, signalmen or rigger or other particulars stated in the permit to work, the old permit to work becomes void and a new permit to work shall then be raised and submitted together with relevant documents as applicable to changes stated in permit.111Use of Mechanical Material Handl

14、ing Equipment (6) Permit to Work System (Contd):q During the course of work, if there are any serious safety violations or non-compliances with conditions stated in permit, EHS shall stop the contractor from performing any further work and work shall not commence until corrective actions are impleme

15、nted. A new permit to work shall be raised and approved by AMD EHS prior to restarting work.q If work is performed without the permit to work, it will be treated as non-conformity.q All the relevant documents shall be attached together with permit to work for approval by AMD EHS Section. 112Work inv

16、olving use of Scaffolds (1) General Safety Requirements:q Use of loose planks as working platform shall be strictly prohibited as there is a potential for serious accidents. Always proper working platform or scaffold shall be used for work.q Prior to climbing and using scaffolds, user shall check on whether signage is displayed on scaffold stating that scaffold is safe to use.q If not sure check with scaffold su
