2、因 蛋 白 在 大 肠 癌 中 的 表 达 及 意 义 。2. 探 讨 大 肠 癌 伴 日 本 血 吸 虫 病 与 不 伴 有 日 本 血 吸 虫 病 两 组 病 例 中 AKT1，Survivin， caspase-3 三 种 基 因 蛋 白 的 表 达 水 平 的 差 异 ， 以 期 了 解 日 本 血 吸 虫 在 大 肠癌 癌 变 过 程 中 的 作 用 。【方法】运 用 免 疫 组 化 S-P 法 检 测 75 例 伴 与 不 伴 有 日 本 血 吸 虫 病 的 大 肠 腺 癌 、 40 例 伴与 不 伴 有 日 本 血 吸 虫 病 的 正 常 大 肠 粘 膜 中 AKT1、 Surv
3、ivin 及 caspase-3 的 表 达 ， 并分 析 其 与 临 床 病 理 特 征 之 间 的 关 系 ， 以 及 三 者 间 的 相 关 性 。【结果】1. AKT1、 Survivin 分 别 在 正 常 大 肠 黏 膜 、 大 肠 腺 癌 之 间 的 阳 性 表 达 差 异 有 非常 显 著 的 统 计 学 意 义 （ P0.05） 。 Survivin 阳性表达率与患者肿瘤分化程度、 生 存 时 间 有 关 （P0.05） 。【结论】在 大 肠 腺 癌 中 AKT1、 Survivin 呈 高 表 达 ， caspase-3 呈 低 表 达 ， 提 示 AKT1、Survivi
4、n 以 及 caspase-3 在 大 肠 癌 癌 变 过 程 中 发 挥 重 要 的 作 用 ， 促 进 了 大 肠 癌 的 发生 发 展 。 Survivin 阳 性 表 达 率 与 患 者 肿 瘤 分 化 程 度 、 生 存 时 间 有 关 ， 可 以 作 为 预 后评 估 的 指 标 。 大 肠 腺 癌 伴 日 本 血 吸 虫 病 与 不 伴 日 本 血 吸 虫 病 两 组 病 例 中 AKT1、Survivin、 caspase-3 表 达 无 明 显 差 异 ， 提 示 在 大 肠 癌 癌 变 过 程 中 日 本 血 吸 虫 没 有 对这 三 项 指 标 产 生 明 显 影 响 。

7、，in order to detect if schistosomiasis has effect on colorectal adenocarcinoma.MethodsImmunohistochemical S-P staining method was adopted to research the expressionof AKT1、 Survivin and caspase-3 in 75 cases of colorectal adenocarcinoma withschistosomiasis or without schistosomiasis, and in 40 cases
8、 of normal colorectal mucosawith schistosomiasis or without schistosomiasis. we analyzed the relationship of thesecases with their clinicopathologic characteristics, and the relationship among these threefactors.Results1. The difference of positive expression of AKT1 and Survivin was significantbetw
9、een colorectal adenocarcinoma and normal colorectal mucosa(P0.05） ； The positive expression ofSurvivin did appear to relate to histological differentiation and survivaltime(P0.05） .ConclusionThere is high positive expression of AKT1 and Survivin, and low positiveexpression of caspase-3, which sugges
10、ts that these factors are concerned with thedevelopment and progression of colorectal adenocarcinoma. Survivin is ralated tohistological differentiation and survival time, hence it may be contributed to prognosticjudgment.The expression of AKT1 、 Survivin and casepase-3 between colorectaladenocarcin

14、 性 肿 瘤 ， 在 经 济 发 达 国 家 更 为 常 见 ， 而 且 发 病 率有 逐 年 增 多 的 趋 势 。 我 国 也 是 结 、 直 肠 癌 高 发 地 区 之 一 ， 特 别 是 近 20 年 来 ， 随 着人 民 生 活 水 平 的 不 断 提 高 ， 膳 食 中 肉 类 、脂肪 、 纤 维 素 的 结 构 比 例 逐 步 西 方 化 ，大 肠 癌 的 发 病 率 也 在 逐 年 增 加 ， 呈 明 显 上升趋 势 ， 尤 其 是 经 济 较 发 达 的 沿 海 城 市和 东 部 地 区 ， 其 上 升 趋 势 更 为 显 著 。随 着 分 子 生 物 学 研 究 的 发 展
15、 ， 对 日 本 血 吸 虫 与 宿 主 之 间 的 关 系 有 了 新 的 认 识 。血吸虫在宿主体内成熟，虫卵分泌可溶性虫卵抗原，在虫卵周围诱导产生主要由 I型细胞因子依赖的 T 细胞介导的肉芽肿反应。S oliman 等 1在研究 59 例大肠癌患者的分子病理机制时发现，血吸虫病常伴有微卫星不稳定性及 ras 基因突变，提示血吸虫病在大肠癌的发生过程中是一种促进因子。Zhang 等 2也检测了 44 例患有直肠癌 的 中 国 人 P53 基 因 的 突 变 情 况 ， 其 中 22 例 伴 有 日 本 血 吸 虫 病 。 与 非 血 吸 虫 性 肠 癌相比，血吸虫性肠癌中 CPG-二核苷
16、酸配对替代的比例更高，但差别没有统计学意义 。 血 吸 虫 性 肠 癌 中 精 氨 酸 错 义 突 变 比 非 血 吸 虫 性 肠 癌 更 常 见 ， 且 248 号 密 码 子 突变 的 频 率 更 高 ， 因 而 表 明 日 本 血 吸 虫 感 染过程 中 可 能 产 生 内 源 性 基 因 毒 性 因 子 引起 血 吸 虫 性 肠 癌 中 的 基 因 突 变 。 上 述 研 究结果 为 深 入 探 讨 血 吸 虫 是 否 导 致 组 织 异常 增 生 并 促 发 大 肠 癌 提 供 了 科 学 理 论 依 据 。大 肠 癌 的 发 生 、 发 展 、 浸 润 以 及 转 移 非 常 复 杂 ， 与 多 种 癌 基 因 和 抑 癌 基 因 的表 达 、 调 控 以 及 细 胞 凋 亡 密 切 相 关 。 对 大肠癌 各 个 阶 段 的 不 同 基 因 表 达 产 物 进 行观 察 ， 在 肿 瘤 的 基 础 与 临 床 研 究 中 有 重 要 意 义 。 AKT 是 PI3K-AK
