1、RENAL评分系统在肾癌肾部分切除术中的应用 lR.E.N.A.L.评分系统是由KutikovKutikov等等【1 1】在2009年第一次提出的，该系统是一个术前肾肿瘤评分系统。l该系统对预行保留肾单位手术的患者基于肾肿瘤影像学解剖特征进行术前肾肿瘤综合评估，客观评价肾脏肿瘤的手术难度以达到指导临床医师合理选择手术方式和治疗策略的目的。R.E.N.A.L.评分计算方法l对每一变量进行量化评分后，每组得分按首字母缩写的顺序相加得到总分。R.E.N.A.L.评分举例（左肾上极肿瘤）R.E.N.A.L.评分举例l肿瘤大小4.1cm=2分l外凸率50%=2分l距肾盂距离9分时，开放手术的比例明显多于
2、腹腔镜手术。R.E.N.A.L.评分系统与手术方式相关性l国内刘强等【4】研究表明：在临床上，患者的RENAL的评分越低，临床医生更倾向于肾部分切除术术式，反之倾向于肾根治性切除术。这与国际上很多的报道结果一致【5-6】。该研究也显示，与术式的选择存在直接关系的是年龄、R、N、L， R、N、L3项的单项的得分越高及患者的年龄越大，更倾向于采用RN术式，反之则倾向于PN。R.E.N.A.L.评分与围手术期并发症关系l存在争议lCanter等【3 3】在证实R.E.N.A.L.评分系统与手术方式具有显著相关性的同时也认为该评分系统与围手术期并发症无显著相关性。lHew等【7 7】认为R.E.N.A
3、.L.评分9分是发生围手术期并发症的高危因素。R.E.N.A.L.评分与术后并发症发生率之间的相关性lBruner等【8 8】报道称，R.E.N.A.L.评分每增加1分，术后尿漏的概率增加35%。lMathieu等【9 9】认为R.E.N.A.L.评分与术后并发症无相关，外科医生的经验、出血和集合系统开放是3个术后并发症预测因素。R.E.N.A.L.评分系统对肾部分切除术后保留肾单位GFR的预测l存在争议lSimmons等【1010】研究显示肿瘤评分与功能肾单位保留紧密联系，是决定肾部分切除术后肾功能的重要因素。lMufarrij等【1111】通过比较R.E.N.A.L.评分低、中、高3组肾部
