11、ion of production safety and the Olympic Games and other学生学分认定管理办法根据教育部2003年颁发的普通高中课程改革方案(实验)和山东省普通高中学生学分管理方案(试行)的要求，为科学、规范、有序实施普通高中学生学分的认定和管理，促进高中新课程的顺利推进，结合我校的实际情况，特制定本管理方法。 一、学分的意义 普通高中课程改革方案的精神阐明，学分是对学生在课程学习方面的经历和发展水平的认定，用以记录学生在相应的课程领域的成长经历和达到的发展程度。为了更好地在新课程实施中呈现构建意义上的学习评价，在传统甄别的基础上，向评价主体的多元化，评价
12、内容的多重性，评价方法的多样化拓宽，使学生学业评价真正成为真实描述与反映学生学业发展历程和状况的过程，自我教育的过程。 二、学习模块学分认定的基本内容及方法 1、学习课时。学校必须按照课程计划规定的课时开设课程，学生原则上都必须全程参加课时学习。学生参加课程学习时间必须达到规定的80%以上。全部学生学习课时由任课教师认真登记，2 学习过程反映（60分以上为合格）（1）听课状态。任课教师对学生在课堂上的听课情况进行量化打分，总分为20分，实行扣分制，根据学生的表现酌情扣分。（2）预习完成学案情况。任课教师对学生的预习情况进行量化打分，总分为15分，根据学生的预习情况酌情扣分。（3）作业情况。任课
13、教师对学生的作业情况进行量化打分，总分为20分。（4）合作学习。任课教师对学生在课堂上的参与、讨论等作出评价，进行量化打分，总分为20分，实行扣分制。（5）迟到情况。任课教师对学生每一节的迟到情况进行记录。无迟到现象得10分，每迟到一次扣1分。（6）课堂上及自习上的其他情况：若无下列现象得15分。课堂上睡觉。学生如果在课堂上有睡觉现象，每次扣0.5分；老师提醒后仍然有睡觉现象，每次扣1分。课堂上说话。学生如果在课堂上谈论与学习无关的事情或嘴频等现象时，每次扣0.5分。课堂上看与本节内容无关的书籍。学生如果在课堂上看与本节内容无关的书籍或武打小说、网络小说、艳情小说等，发现一次扣1分，并没收。课
14、堂上玩手机。每发现一次扣1分，并没收手机。突发事件。若学生在课堂上不服从教师管理，出现顶撞老师现象，每次扣2分，并写出书面检讨；情节严重者，送交政教处处理。3、模块考试成绩。每个模块学习结束，由学校根据课程标准统一命题组织终结测试。测试除书面笔试外，应根据学科的特点举行必要的非书面笔试，如英语的听力，口语表达，自然学科的实验操作，体育、艺术学科的表演等。第1、2、3方面全部合格给予相应模块学分。学生因学分认定考试成绩不合格而不能获得学分的，可以申请补考或申请参加其他教学班相同学习模块的考试。补考时间由学校安排。补考或参加其他教学班相同学习模块的考试不得超过2次，仍不及格者，可申请重修、改修或放
15、弃（必修课程和有必修学分要求的选修课程不得放弃，选修课程中要求获得必修学分的模块有其他选择的除外）。重修要在接到不认定学分的通知后1年之内完成，重修所用时间不得与先前修习所用时间累加。同科课程（模块）重修不得超过两次。学生因修习时间不足不能获得学分的，可在补足修习时间后认定。三、综合实践活动学分认定 1、研究性学习学分认定研究性学习主要围绕音乐、美术、体育等方向开展，也可参与兴趣小组、社团等形式认定。参与音乐、美术、体育等方向的主要抓好两个阶段：第一阶段，学习过程评价。第二阶段，测试成绩（由指导教师评价）。参与兴趣小组、社团等形式的，由指导老师评价、学生自评、互评综合评定。重点抓好三个阶段:第
16、一阶段，计划阶段。参与的学生写出学习计划或活动计划。第二阶段，学习过程评价。学习课题组及学生本人提供每次研究学习活动记录，课题研究中所收集的材料(包括原始材料)。第三阶段，总结阶段。学习兴趣小组、社团的学生本人提供具体反映本人参与研究学习的感受、体会、小结，由指导教师评价。 2、社区服务学分认定 社区服务和社会实践的评价由政教处组织评定。学生在三年内应参加不少于10个工作日内的社区服务，共获得2个学分。参加的社区服务少于10个工作日，不给学分。公益性是社区服务的最大特色，服务者的志愿与义务与否，是判断某种活动是否社区服务活动的标准。学生完成社区服务的必须向学校提供相关的经服务单位认可的社区服务材料，得以评定学分。学生可以从学校提供的社区服务清单中选择服务内容，也可以自己申报，经学校审批备案。社区服务以小组或行政班的方式进行。学校依据既定
