11、点 2008-01-08 13:57:391 目的和范围 1.1 为确保XXX审核人员对组织职业健康安全管理体系进行有效审核，特制定本工作指导书。1.2 适用于XXX各级审核人员认证过程中对组织职业健康安全管理体系进行审核。2 引用文件2.1 CNAS-CC41职业健康安全管理体系认证机构通用要求2.2 CNAS-CC42职业健康安全管理体系认证机构通用要求应用指南2.3 CNAS-GC41职业健康安全管理体系认证机构认证业务范围管理实施指南2.4 GB/T28001:2001职业健康安全管理体系 规范3 职责3.1 审核组负责本工作指导书的实施。4 指导内容4.1 职业健康安全管理体系审核由
12、两个审核阶段组成，两个阶段的审核目的各有侧重。4.1.1 第一阶段审核目的：通过对受审核方的管理权限、活动领域和现场区域等各个方面进行了解与观察，确定第二阶段的审核范围；通过对若干核心要素与标准符合性的审核，以及受审核方对审核中所发现问题的整改情况，确定第二阶段审核的可行性和条件；通过对组织所建立的文件化、结构化管理体系的了解，包括危险源及其相关风险、体系的策划和自我完善机制的实施状况，确定第二阶段的审核重点。a） 重点审核体系的策划情况，有关运行情况只深入到“做了没有”；b） 文审部分应着重评审组织是否按标准要求对相关要求进行了有效的策划；c） 通过现场审核，应确定第二阶段审核所需资源的配置
13、情况，如审核组的人员组成、审核组的人员专业能力、所需的审核人日数等要求；d） 基于审查结果，综合评价组织管理体系的实施程度，判断是否为第二阶段审核做好了准备，并与组织有关人员进行讨论，商定第二阶段审核的细节。4.1.2 第二阶段审核目的：证实受审核方实施了其方针、目标指标，并遵守了职业健康安全管理体系的各项相应程序；证实受审核方的职业健康安全管理体系符合标准的所有要求，能够实现其方针和目标；评价受审核方的职业健康安全管理体系是否符合审核准则，以决定其是否可以通过职业健康安全管理体系认证并推荐注册。a） 审查管理体系与相应的管理体系标准的所有要求的符合情况，并要求提供相应证据，评价“做得如何？”
14、，符合性和有效性如何？b） 体系范围包括管理层、各部门、车间，运行现场都要逐一进行审核；c） 相应管理体系标准的全部条款和要点都要逐项进行审查。4.2 审核内容安排4.2.1 第一阶段重点审核内容主要涉及4.1、4.2、4.3、4.5.4和4.6。a） 审核4.1是为了总体评价组织是否按标准建立和实施了体系运行，并确定其审核范围；审核4.2和4.3是要摸清组织的重大风险及其法律法规，从而确定方针、目标、措施和管理方案的适宜性和合理性。因此，这几个标准条款一般要在第一阶段审核到位，否则，就会出现由于重大风险识别不全，目标指标和管理方案不合理，导致各层次、各部门的目标和控制失控。到第二阶段审核时，
15、就难以获取评价职业健康安全绩效以及实现目标能力的证据，给管理体系有效评价带来困难。b） 有关资源、作用、职责和权限、运行控制、应急准备和响应以及监测和测量，在第一阶段不作全面细微的审核。其中4.4.1只要在现场确认组织机构和职责，有助于第二阶段审核的策划；而对4.4.6、4.4.7和4.5.1主要是了解有关程序是否建立并开始进行，即弄清楚准备情况，具体的运行实施情况，在第二阶段再作全面细微的审核，以确定其有效性。4.2.2 根据第一阶段已重点审核的结果，第二阶段需进一步补充审核与条款4.1、4.2、4.3、4.4.1、4.5.4和4.6有关的内容，条款4.4.24.4.7和4.5.14.5.3
16、在第二阶段都必须全面系统的审核。4.2.3 通过上述安排，明确区分了两个阶段的审核内容和方法，能实现两个阶段的审核目的，也能解决两个阶段审核的重复问题和某些条款审核不到位的问题，从总体上能够提高审核的绩效和认证的有效性。4.3 通常情况下，第一阶段审核应在组织现场进行，以下情况可考虑不到现场进行：4.3.1 ISO17021：2006标准9.2.3.1.1条规定：“对于大多数管理体系而言，建议至少部分第一阶段审核活动在客户的场所进行”，这是由第一阶段审核目的所决定的，为了实现第一阶段审核目的要求安排的审核内容，大部分只有到现场才能准确获得相关信息。但是对于一些个案，可以不到现场进行第一阶段审核。4.3.2 集团公司下设的各个分公司
