15、应商名单进行审核。4.5.3负责对合格供应商名录中的主要供应商进行审核。5 程序5.1 供应商名录的建立5.1.1供应商名录来源于全公司范围内各种渠道的信息征集，收集掌握各类设备材料潜在供应商及其基础信息。5.1.2供应商名录经合格供应商选择、评价流程通过后方能增补进合格供应商名录。5.2 新供应商选择5.2.1采购部组织工程管理部/特许经营管理部、设计部对供应商进行选择，包括对供应商规模、资质、技术和业绩的选择。5.2.2采购部负责供应商规模、资质的选择，供应商规模、资质包括：营业执照、税务登记证、一般纳税人资格证明、质量体系认证，财务报表，特殊物资供应商还需提供物资生产许可证、物资经销许可
16、证、物资运输许可证、物资安装许可证、计量器具许可证、国家环保局审批证及代理授权书等。采购部评审完毕后形成书面意见。5.2.3工程管理部/特许经营管理部和设计院负责对供应商资质、业绩和设备技术能力的选择。工程管理部/特许经营管理部和设计院评审完毕后形成书面意见提交采购部。5.2.4采购部依据上述评审意见评价供应商，建立合格供应商名录。5.3合格供应商名录的维护5.3.1合格供应商名录收录的供应商，必须经过评价。其所收录设备材料种类应覆盖业务单元的实际需要；其中每类物资供应商原则上不能少于3家，否则应说明情况。5.3.2合格供应商名录的审核，一般按专业划分，集中会议的方式进行。具体由采购部负责汇总收集和整理评价资料，由招标委员会统一进行综合审核。5.3.3合格供应商名录由采购部根据审核结果汇总编制后发布实施。5.3.4评价意见
