1、船/岸安全检查表SHIP/SHORE SAFETY CHECK LIST船名： 泊位： 港口ShipName： Berth No.： Port： 靠泊时间： 离泊时间： Time of arrival: Time of depart： 填表说明： 为了安全作业，对所有的问题均应肯定地做出回答，可在相应的方格内清 楚地标上“V”记号。如果不能作肯定的回答，则应说明理由，并就所应采取 的相应的措施在船舶与油码头之间达成协议。对认为不适用的任何问题，就在 备注栏中注明。“船舶”和“油码头”栏目下的方格，是供双方各自填写检查结果的。 在“代码”栏目中标有字母A、P或R者，分别表示如下意义：A任何议定的
