1、 医保,检验报告单用药篇一：2015医保定点药房自检自查报告 仁心大药房医保定点药房自检自查报告 为贯彻落实云人社通?2015?100号文件精神，根据省、州、县人力资源和社会保障局要求，结合云南省基本医疗保险药品目录、云南省基本医疗保险诊疗项目、云南省基本医疗保险服务设施标准的标准，仁心大药房组织全体员工开展医保定点药房自检自查工作，现将自检自查情况汇报如下： 一、本药房按规定悬挂定点零售药店证书、公布服务承诺、公布投诉电话，营业执照、药品经营许可证、药品经营质量管理规范认证证书均在有效期内； 二、本药店有严格的药品进、销规章制度，药房员工认真履责，对首营企业和首营品种认真审核并建立档案，确保
2、购货渠道正规、合法，帐、票、货相符； 三、本药房营业时间内至少有一名药师在岗，药房门口设有明显的夜间购药标志。所有营业人员均持有相关主管部门颁发的上岗证、健康证和职业资格证书，且所有证书均在有效期内，药师按规定持证上岗； 四、本药房经营面积284平方米，共配备4台电脑，其中有3台电脑装药品零售软件，1台装有医保系统，并经专线连入怒江州医保系统。配有相应的管理人员和清洁人员，确保了计算机软硬件设备的正常运行和经营场所的干净整洁； 五、本药房药学技术人员按规定持有相关证件。所有员(来自:WWw.cssyq.Com 书业网:医保,检验报告单用药)工均已购买社会保险；药房严格执行国家、省、州药品销售价

5、隔不得少于15天（急诊、化疗除外）； 7.按照规定出院带药（剂型、剂量）; 8.医生是否开具与患者疾病无关的药品、检查、治疗； 9.患者用药中目录内药品使用率不得低于85%； 10.住院药费占医疗总费用<=50%； 11.住院药费中甲类药品占药品总费用>=30%,自费药品占药品总费用<=15%; 12.检查检验费用占总医疗费用<=35%; 13.住院期间外检项目需有客观临床依据、医嘱、病程记录、报告单； 14.自费项目按知情同意原则，需征得参保人员或其家属同意并签字（急诊急救除外）； 15.处方应书写规范、清晰、准确、完整，开具的西药处方需符合西医疾病诊断原则；开具中（
