5、 摘 要雌激素水平与绝经后女性冠状动脉病变相关性分析摘 要目的 探讨雌激素水平等因素与绝经后女性冠心病患者冠状动脉病变的相关性。方法 采用病例对照研究法，收集因疑诊冠心病而收住入院的绝经后女性患者（绝经 时 间 1 年 ） ， 根 据 冠 状 动 脉 造 影 结 果 分 为 冠 心 病 组 （ 68 例 ） 及 非 冠 心 病 组 （ 41 例） ，查 雌 激 素 、 孕 酮 、 血 糖 、 血 脂 、 尿 酸 、 纤 维 蛋 白 原 及 血 红 蛋 白 ， 并 收 集 传 统 冠 心 病 危 险因 素 进 行 比 较 。 并 且 根 据 冠 脉 病 变 支 数 及 冠 脉 病 变 Gensi
6、ni 积 分 评 估 结 果 进 行 分 组 ， 评价雌激素水平与绝经后女性冠脉病变的关系。结果 冠心病组较非冠心病组年龄大，高血压、高脂血症发病率高，而雌二醇、血红蛋白水平低（P 0.05） 。 非 条 件 Logistic 回 归 显 示 ， 高 血 压 及 高 甘 油 三 酯 为 女 性 冠心 病 的 危 险 因 素 （ OR 值 分 别 为 7.884、 7.34） ， 雌 二 醇 与 血 红 蛋 白 为 女 性 冠 心 病 的 保 护因 素 （ OR 值 分 别 为 0.905 及 0.853） 。 但 雌 二 醇 及 孕 酮 水 平 在 冠 脉 病 变 单 支 、 双 支 及 多支
7、 组 中 的 差 异 无 统 计 学 意 义 ， 并 且 在 冠 脉 病 变 Gensini 积 分 0 的 各 组 中 雌 二 醇 及 孕 酮水 平 的 差 异 也 无 统 计 学 意 义 。结论 绝 经 后 女 性 随 着 年 龄 增 加 、 雌 二 醇 水 平 下 降 、 雌 孕 激 素 比 例 失 调 ， 冠 心 病 发病率呈上升趋势。绝经后女性冠心病是由多重危险因素综合而致，其合并高血压、高脂血症者多，并且血红蛋白水平低于未患冠心病者，而且随着绝经时间延长，冠脉病变程度逐渐加重，以多支病变为主。雌二醇是女性冠心病的保护因素，但它与绝经后女性冠脉病变程度却未见明显相关性。孕酮水平高低对

9、s in postmenopausal women with coronary heart disease.Methods This is a case-control study which involved postmenopausal women（ menopause 1year） undergoing coronary angiography, including 68 with confirmedcoronary heart disease(CHD)and 41non-CHD patients.The empty stomach level of estrogen(E2), prog
10、esterone(P), lipid, blood glucose， UA， FIB,HGB and the traditional coronary riskfactors were investigated in two groups. And according to the vessel disease and Gensiniscores,all patients were divided into four groups,to evaluate the relationship between estrogenlevels and coronary artery lesions in
11、 postmenopausal women.Results The age ,hypertension and hyperlipidemia increased,the level of E2 and HGBdecreased in the CHD group compared with the non-CHD group(P 0.05） The logisticregression results showed that hypertension and hypertriglyceridemia are risk factors(OR=7.884、 7.34)， E2 and HGB are
12、 protective factors for CHD in women(OR=0.905、0.853).But in the single,double and multi vessel disease groups,the E2 and P levels were nosignificant difference.There has the same result in each of groups that Gensini scores 0.Conclusion The morbility of CHD with the age and the lower estrogen levels
13、 inpremenopausal women, often increased with multiple risk factors, the more risk factors ofthem are hypertensive, hyperlipidemia and hypohemia. As the postmenopausal timeextended,the level of coronary lesions gradually increased with multi vessel disease.Theestrogen is a portective facter for coron
