2、 较 注 射 hCG 日、移 植 （ ET） 日 以 及 黄 体 中 期 血 清 雌 激 素 （ E2） 的 水 平 ， 了 解 E2 水平或 下 降 程 度 对 妊 娠 结 局 的 影 响 ， 探 讨 雌 激 素 在 IVF-ET 黄体支持中的 作 用 ， 从 而 为 制 定 最 简 单 、 有 效 的 黄 体 支 持 方 案 提 供 依 据 。方法：1. 回 顾 性 分 析 785 个 IVF-ET 治 疗 周 期 ， 其 中 384 周 期 仅 予 黄体 酮 80mg/d 黄 体 支 持 ， 401 周 期 给 予 黄 体 酮 80mg/d +补 佳 乐 （ 戊 酸雌 二 醇 ） 2mg
3、/d， 比 较 黄 体 支 持 中 补 充 小 剂 量 雌 激 素 对 妊 娠 率 的 影响 。2. 回 顾 性 分 析 黄 体 期 未 补 充 雌 激 素 的 384 周 期 ， 按 hCG 日 E2值 （ 单 位 pg/ml） 分 ： 2000 组 、 20004000 组 、 4000 组 ； 按ET 日 E2 值 （ 单 位 pg/ml） 分 组 ： 500 组 、 5001000 组 、 1000组 ； 按 hCG 日 E2 与 ET 日 E2 比 值 分 组 ： 4.0 组 、 4.0 组 ， 了解 E2 峰 值 水 平 及 取 卵 后 E2 下 降 程 度 对 IVF 妊 娠 率
4、 的 影 响 。3. 测定 IVF 取 卵 后 因 某 种 原 因 未 行 移 植 者 黄 体 中 期 E2 水 平 ， 与自 然 周 期 黄 体 中 期 的 E2 水 平 比 较 ， 对 实 验 的 第 一 、 二 部 分 结 论 作 进一 步 的 实 验 论 证 。结果：1. 补 充 组 与 未 补 充 组 比 较 ： 两 组 在 年 龄 、 获 卵 数 、 受 精 数 、 种植 率 、 临 床 妊 娠 率 均 无 统 计 学 差 异 (P 0.05)；2. 未补充组按 hCG 日 E2 水 平 比 较 ： 各 组 间 在 获 卵 数 、 受 精 数 上有 显 著 差 异 （ P 0.01
5、） ， hCG 日 E2 水 平 4000 组 获 卵 数 、 受 精 数 最多，但各组种植率、临床妊娠率无统计学差异 (P 0.05)；按 ET 日E2 水平比较， E2 水平 1000 组 获 卵 数 、 受 精 数 明 显 高 于 其 他 组 （ P0.05)， 但 三 组 种 植 率 、 临 床 妊 娠 率 也 无 统 计 学 差 异 (P 0.05)； 按 hCG日 与 ET 日 E2 比 值 分 组 ， 发 现 两 组 的 获 卵 数 、 受 精 数 、 种 植 率 、 临 床妊 娠 率 上 差 异 均 无 显 著 性 (P 0.05)；3. IVF 取 卵 后 黄 体 中 期 E
6、2 水 平 明 显 高 于 自 然 周 期 者 （ P 0.05） 。结论：黄 体 期 补 充 小 剂 量 雌 激 素 进 行 黄 体 支 持 并 不 能 改 善 IVF 种植率、临 床 妊 娠 率 ， 所 以 黄 体 期 补 充 雌 激 素 是 非 必 需 的 。 hCG 日 E2 水平、取 卵 后 黄 体 早 期 E2 下 降 程 度 及 黄 体 中 期 E2 水 平 均 不 影 响 IVF 妊娠率。关键词：体外受精 -胚 胎 移 植 ， 雌 激 素 ， 黄 体 支 持 ， 临 床 妊 娠 率THE ASSOCIATION OF SERUM ESTRADIOL ANDPREGNANCY O
7、UTCOME FOLLOWING IN VITROFERTILIZATION-EMBRYO TRANSFERABSTRACTObjective:To retrospective analysis IVF-ET treated cycles, Comparing thepregnancy rates (PRs) between the progesterone alone and estrogen plusprogesterone as luteal support, Studying the effect of serum estradiol (E2)levels on the day of
8、hCG administration(hCG-day)、 the day of embryotransfer (ET-day) and the mid-luteal, to determine the relationshipbetween the serum E2 levels and the outcome of IVF, to know the effectof E2 in luteal support. So as to find a simple、 impactful way of lutealsupport.Method:1. 786 IVF-ET cycles were eval
9、uated in this study. 384cyclesreceived progesterone alone (80mg/d), 401 cycles received progesterone80mg/d and estrogen 2mg/d, to evaluate the relation of Estrogensupplementation and PRs.2. Analyzed 384cycles received progesterone alone, divided it intogroups according to E2（ pg/ml） of hCG-day: 2000
10、 group、 20004000group、 4000 group； according to E2 （ pg/ml） of ET- day： 500 group、5001000 group、 1000 group； according to the ratio of hCG-day E2 toET- day E2： 4.0 、 4.0， compared these groups to know if the peaklevel of E2 and markedly declined E2 effect the outcome of IVF.3. compare the E2 of mid-
11、luteal between IVF cycles which gave upET after retrieving oocytes and nature cycles.result:1. Compared the progesterone alone and estrogen plus progesteroneas luteal support,we found there was no statistical significance in age、number of mature oocytes retrieved、 number of oocytes fertilized、implan
12、tation、 PRs(P 0.05).2. The group received progesterone alone was divided according toE2 of hCG-day:We found the group E2 4000 had more mature oocytesretrieved、 oocytes fertilized（ P 0.01） , but there was no statisticalsignificance in implantation、 PRs(P 0.05). Compared the groups dividedaccording to
13、 E2 of ET-day:We found the group E2 1000 had more matureoocytes retrieved、 oocytes fertilized（ P 0.01） , and the implantation、PRs has no statistical significance(P 0.05).Compared ratio of hCG-dayE2 to ET- day E2: the two groups have no statistical significance in age、number of mature oocytes retriev

E2 of mid-luteal after retrieving oocytes in IVF cycles obviouslyhigher than nature cycles .Conclusion:There was no beneficial effect by adding estrogen to progesteronefor luteal support in IVF-ET cycle， so add estrogen during the lutealph
