1、成都中医药大学 硕士学位论文四川名老中医杜自明骨伤学术思想及治疗经验的总 结 姓名：张晓剑 申请学位级别：硕士 专业：中医骨伤科学 指导教师：江蓉星 2009- 04 中文摘要目的：1、在骨科临床中，中医骨伤科许多行之有效的治疗方法筋伤 的中医手法治疗、体功锻炼、中药内服与外敷等治疗方法或被忽略遗忘或未完全继 承导致疗效差，鉴于此本文较完整的发掘出名老中医杜自明骨伤学术思想及治疗经 验的精髓，并继承、提炼出杜派治疗筋伤、骨伤的独特治疗方法，以指导目前教学、 科研、临床工作；2、详细总结杜派治疗筋伤的手法，以期更有利于学习、继承、掌握 和发扬光大； 3、研究杜派推拿手法配合中药内服治疗神经根型颈
2、椎病的机制和疗 效；4、数据库建设。方法：1、通过调查研究、专家咨询、文献查阅与检索、整理分析总 结原始资料，继而跟随杜派传人学习，并与杜派传人研讨，总结资料；2、回顾性研究： 治疗组运用杜派推拿手法配合中药内服治疗神经根型颈椎病 66 例，对照组予以枕 颌带牵引配合中药内服治疗神经根型颈椎病 40 例，观察两组的疗效。结果：1、结合 个人在整理过程中的体会，以图片的形式展示了医生练功和患者体功锻炼的自我保 健方法；2、杜派中医骨伤学术思想及治疗经验内容丰富，杜派尤以手法治疗骨伤和 筋伤为特色；3、杜派推拿手法配合中药内服治疗神经根型颈椎病能有效的改善患者 的临床症状及体征（Plt0.01），
3、延缓病情发展，并且无明显不良反应。结论：1、杜派中 医骨伤学术思想是中医骨伤科的文化瑰宝，其治疗经验是中医骨科治疗中不可缺少 的一部分，应予以继承和发扬光大；2、杜派推拿手法配合中药内服治疗神经根型颈 椎病疗效肯定，并且无明显不良反应，值得推广。关键词：杜自明 中医骨伤 学术思 想 治疗经验 手法 治疗 颈椎病 临床研究 AbstractObjective: 1. 15、 痛、肩关节、肘关节、腕关节、髋关节、膝关节、踝关 节的扭伤。5、详细论述了杜派推拿手法的发展情况。6、通过展示目前临床中运用小 夹板治疗肱骨髁上骨折、桡骨远端骨折的详细步 骤，以体现杜派骨科治疗骨折的特 点。 第一部分1 四川名老中医杜自明简介 杜自明（7）满族，中医正骨 专家。出生于四川省成都市一个正骨世家，随父临症，学习骨伤病的诊疗技术。杜氏 天资聪慧，勤奋好学，曾多次外出寻师访友，在一些有一技之长的拳师或民间正骨医 生的门下虚心求教，对不同流派的正骨技法，做到兼收并蓄，丰富和提高了自身医学 理论和理伤正骨 1 成都中医药大学硕士学位论文技术，并以济世救人为宗旨。杜氏 少年时喜爱武术，习
16、练十八般兵器，宗少林派武功，以弄拳、击剑、舞刀见长，尤擅猴 拳，每日坚持“易筋经十二段”的锻炼，外练筋骨，内练精气。1902 年开始，以行医为 业，中年时代就已驰名于成都武术界和骨伤科行业。1950 年成都市特聘两名骨伤科 专家，杜氏和朱海 2 人入选，并颁发了聘书和奖金。年，卫生部组建中医研究 院时，曾广泛邀聘全国各地知名中医专家参加中医的整理研究工作。杜自明作为四 川著名正骨专家被正式聘请到京，出任中医研究院内外科研究所骨科主任。在北京 工作期间，他虽然年迈八旬，依然壮心不已，对中医事业的前景充满信心，把全部精 力投入到正骨临床工作，总结出许多中医骨伤的治疗经验，并毫无保留地把多年积累的正骨经验传给下一代，培养了多名正骨人才。先后被选为市人民委员会委员和 市人民代表。并于 1959 年担任第三届全国政治协商委员会委员，当年还荣获“全国 卫生先进工作者”称号。同年由他口述，经其弟子整理，出版了中医正骨经验概述 一书，1960 年拍摄了杜自明正骨经验科教电影片，还编写
