6、 市 中 心 医 院 病 理 科 )摘 要 目 的 :探 讨 乳 腺 浸 润 性 微 乳 头 状 癌 ( IMPC)的 病 理 形 态 学 特 点 、 诊 断 、 鉴 别 诊 断 和 预 后 。 方 法 :采 用 光 镜 和免 疫 组 化 SP法 对 3例 IMPC进 行 观 察 并 结 合 文 献 分 析 。 结 果 : 3例 乳 腺 IMPC与 浸 润 性 导 管 癌 相 比 无 特 殊 的 临 床 症 状 、体 征 及 大 体 检 查 。 光 镜 下 特 征 性 表 现 为 肿 瘤 细 胞 呈 桑 椹 状 、 微 乳 头 状 或 小 腺 管 样 排 列 ,癌 巢 与 周 围 间 质 形
7、成 明 显 的 空隙 ,微 乳 头 缺 乏 纤 维 血 管 轴 心 ,每 个 微 乳 头 细 胞 团 和 周 边 的 纤 维 组 织 均 存 在 无 细 胞 的 间 隙 样 结 构 ;瘤 细 胞 CerbB - 2、 CgA和 EMA ( + )。 结 论 :浸 润 性 微 乳 头 状 癌 是 一 种 少 见 类 型 的 浸 润 性 乳 腺 癌 ,具 有 淋 巴 管 侵 袭 性 强 、 淋 巴 结 转 移 率 高 、 预 后差 的 生 物 学 行 为 。关 键 词 乳 腺 肿 瘤 ;微 乳 头 ;浸 润 癌 ;免 疫 组 化C lin icopa tholog ica l Fea tures

9、ical characteristics, diagnosi, differential diagnosis and p rognosis of invasivem icropap illary carcinoma ( IMPC) of the breast. Methods: Immunohistochem ical SP method and light m icroscopy were adop ted tomake an observation and analysis of 3 cases of IMPC, based on literatures. Results: Compare
10、d with infiltrating ductal carcinoma,3 cases of IMPC were not significantly different in clinical symp tom s, signs and general check - up. W hat was observed under alight m icroscope included: tumor cells were mulberry and m icropap illary shaped or were of glandule tubular arrangement; therewas ob
11、vious interspace between cancer nest and neighboring areas; m icropap illary was emp ty of fiber blood vessel axes, with m i2cropap illae floating freely in spongy spaces and separated by fibrous sep ta. Tumor cells exp ressed c - erbB - 2, CgA and EMA.Conclusion: IMPC is a kind of rare invasive bre

13、 癌 ( invasive m icropap illarycarcinoma, IMPC) ,以 往 被 作 为 浸 润 性 导 管 癌 的 一 种特 殊 形 态 学 表 现 ,近 年 来 由 于 其 生 物 学 行 为 表 现 与 一般 浸 润 性 导 管 癌 不 同 ,具 有 明 显 的 淋 巴 管 侵 袭 性 、 高 淋巴 结 转 移 率 和 预 后 差 等 特 性 ,故 引 起 国 内 外 越 来 越 多学 者 的 重 视 1 - 3 。 2003年 WHO乳 腺 癌 新 分 类 将 其 归为 乳 腺 上 皮 性 肿 瘤 独 立 一 型 。 本 组 报 道 3例 ,并 结 合文 献
14、分 析 其 临 床 病 理 学 特 点 。1 资 料 与 方 法1. 1 临 床 资 料 本 组 3例 均 为 女 性 ,患 者 年 龄 4169岁 ,平 均 57. 6岁 ,均 以 发 现 乳 腺 肿 块 为 主 诉 就 诊 ,病 程 20天 1年 。 肿 块 位 于 乳 腺 外 上 象 限 2例 ,位 于内 上 象 限 1例 ,乳 头 无 明 显 下 陷 。 3例 同 侧 腋 窝 均 可触 及 肿 大 的 淋 巴 结 。 行 乳 腺 癌 改 良 根 治 切 除 术 2例 ,行 肿 块 单 纯 切 除 术 1例 。1. 2 方 法 IMPC 3例 , 1例 为 包 头 铁 路 医 院 手 术
15、 切 除标 本 , 2例 为 包 头 市 中 心 医 院 手 术 切 除 标 本 ,经 4%甲醛 液 固 定 ,常 规 脱 水 ,石 蜡 包 埋 切 片 , HE染 色 光 镜 观察 ;免 疫 组 化 检 查 一 抗 为 上 皮 膜 抗 原 ( EMA ) ,细 胞 角蛋 白 ( CK) , 雌 激 素 ( ER ) , 孕 激 素 ( PR ) , 嗜 铬 素(CgA ) ,癌 基 因 (CerbB - 2) ,采 用 SP法 ,试 剂 购 自 福州 迈 新 生 物 技 术 开 发 公 司 。2 结 果2. 1 病 理 学 检 查 肿 瘤 呈 结 节 状 ,直 径 2. 2 5. 3cm,平
16、 均 3. 8cm。 切 面 呈 灰 白 色 ,质 硬 ,界 限 不 清 ,大多 数 呈 浸 润 性 生 长 ,部 分 肿 瘤 可 见 淡 黄 色 小 灶 状 坏 死 。组 织 学 观 察 肿 瘤 细 胞 排 列 成 微 小 乳 头 状 、 桑 椹 状 或 圆形 、 卵 圆 形 及 不 规 则 大 小 不 等 的 细 胞 巢 团 ,边 缘 粗 糙 毛刺 样 ,乳 头 中 心 缺 乏 纤 维 血 管 轴 心 。 有 些 肿 瘤 细 胞 排列 成 小 腺 管 样 结 构 ,其 边 缘 也 不 光 滑 ,呈 毛 刺 样 。 癌 细胞 巢 周 围 为 细 网 状 纤 维 间 质 ,内 含 丰 富 血 管 和 淋 巴 管 。癌 细 胞 巢 与 周 边 纤 维 组 织 间 有 类 似 腺 管 的 无 细 胞 间隙 ,由 纤 细 的 纤 维 组 织 分 隔 ,见 图 1。 淋 巴 结 内 可 见 相同 形 态 的 转 移 性 癌 组 织
