3、中，由于家庭本身缺乏专业知识和技能，因此在保障老年人健康晚年生活方面，能够提供方便、专业卫生保健服务的社区卫生服务成为实现“老有所医”的重要途径。 但从经济学角度 看，相对于人需求的无限性，资源具有稀缺性。因此，探索有效配置社区卫生资源，保证社区居民，尤其是 作为 社区 重点人群的 居家养老老年人 都能获得公平、高效且令人满意的社区卫生服务是本文的研究目的 。 在研究方法上，首先， 本文 以 2000-2011 年上海统计数据为基础， 利用 洛伦兹曲线、基尼系数和数据包络（ DEA）法， 客观评估 上海社区卫生资源配置的 现状；通过社会调研，获取 上海居家养老老年人对 目前 社区卫生服务满意度
4、的 主观评价，从而总结得出 上海居家养老社区卫生资源配置的现状及其问题 。第二，在探索社区卫生资源配置 的 增量 改革方面：本文运用 加权平均组合模型 ， 预测短期内 居民对社区卫生人力资源的需求量；运用指数平滑 法，预测 物力资源（ 如 床位 ）的 需求量； 通过 建立 数学模型，预测 财力资源（财政投入量） 。 而在存量调整方面：本文以 结构 功能主义和系统理论为 指导 ， 建立社区卫生资源有效配置的概念模型。因此，通过增量改革社区卫生资源和存量调整现有卫生资源，提高社区卫生资源配置的公 平性、利用效率，更好地满足居家养老老年人对卫生服务的需求。 通过上述研究，本文得出的结论是： 第一，目
5、前上海居家养老社区卫生资源配置在总体上是公平的，利用 效率较高 ，且 居家养老老年人对此满意度水平较高，但 各项 资源 的数量配置和资源结构等方面仍有 不足之处。第二， 利用定量研究方法 对上海居家养老社区卫生资源 需求量进行预测后发现，应重视社区卫生服务人力资源、物力资源和财力资源的增量配置。第三，从资源配置的存量调整角度，应 重视人力资源的合理流动 、完善保障和 激励 措施； 注意调整 物力资源的 结构 、II 盘活存量，避免资源浪费和闲置； 同时， 对 各级财政投入的责任与投入机制， 和信息化建设 加以 完善 ， 促进 实现社区居家养老老年人的 健康管理。 本文的创新点在于：第一，评价
6、上海居家养老社区卫生资源配置现状时，采用了主观（即居民满意度评价）与客观（即资源配置的公平性和效率性）相结合的方法，总结得出上海居家养老社区卫生资源配置的现状和存在的问题。第二，从增量改革和存量调整的角度，运用定量和定性结合的方法，对上海居家养老社区卫生资源配置的途径提出对策建议，突破了以往资源配置研究 以 资源配置评估的局限 。 关键词 ： 上海居家养老，社区卫生资源，现状评价分析 ，资源配置方案设计 III COMMUNITY HEALTH RESOURCES ALLOCATION FOR OLD-AGE HOME CARE IN SHANGHAI ABSTRACT Shanghai ha
7、s entered into the aging society since 1979 with rapid speed, especially in recent years. Therefore, in order to protect local senior citizens late life, Shanghai government established and improved the old-age service system including home care, community support for the elderly and institution for
8、 old-age care. Additionally, home-care system was widely accepted by elder people for its own advantages. Therefore, Community Health Service (CHS) plays an important role in providing support to all residents, especially the elder people in the community and ensure their healthy late life. Resource
9、s are limited in comparison with the unlimited demand from people. Therefore, it is significant to allocate CHS recourse in order to offer equal, efficient, and satisfying health service in community. Based on statistic data from 2000 to 2011, quantity analyzing methods were used, including Lorenz c
10、urve, Gini coefficient and DEA; and an evaluation of the current CHS recourse allocation in Shanghai can be made. Combined with a social research, which was applied to get the satisfiction evaluation from the senior citizens, the current situation and problems existing in the CHS resource allocation
11、 can be drawn. By utilizing mathematic methods like weighted average combination forecast model to forecast future CHS resource demand. Based on the Structure-Function theory and Systematic theory, an ideal model was established to find a suitable way to allocate CHS resources and meet the demand of
12、 the home care elders. Conclusions are: firstly, CHS resource allocation in Shanghai is generally equal IV with its high utilizing efficiency and high satisfiction. However, the structure of the resource allocation needs to be improved. Secondly, based on statistics, more attention needs to be place
13、d on enlarge HR, building more CHS institutions and setting more beds and more financial support. Thirdly, directed by Structure-function theories and system theories, emphases need to be placed on HR flowing, encouragement and quality improving; as for material resources, making full use of them ar
14、e significant rather than wasting; encourage social forces to join in establishing CHS information system can be helpful and realize health management in community The innovation of the paper lies in: when evaluating current CHS resource allocations situation, the objective and subjective methods we
15、re combining used in order to find an overall description of CHS resource allocation in Shanghai. Additionally, by combining with quantitative and qualitative methods, raised suggestions to improve CHS resource allocation in Shanghai which can meet home care elder peoples demands.
