1、1胃肠道间质瘤 27 例临床分析作者：汤衍斌，马涛，江小琼，邹远航，付世艳，孙淑明 【关键词】 胃肠道；间质瘤；临床分析摘要 目的：探讨胃肠道间质瘤 (GIST)的临床特点及诊断治疗。方法：收集我院 27 例 GIST 的临床资料进行回顾性分析。结果：本组平均发病年龄 48.6 岁。发生部位分别为：胃 13 例(48.1)，空肠 7 例(25.9) ，回肠 5 例(18.5)，十二指肠、结肠各 1 例(各37 ) 。本组病例术前均未能确诊。 27 例均行外科手术切除，病理证实良性 21 例(77.8) ，恶性 6 例(22.2) 。肿瘤的大小与良恶性相关(P0.05)。术后随访：良性 GIST
2、 18 例，平均随访时间246 个月， 16 例无局部复发、转移；2 例术后 1 年内复发再手术；恶性 GIST 6 例，平均随访 264 个月，3 例局部复发再手术，1 例肝肺转移：2 例死亡。 5 例恶性 GIST 术后辅以不规则化疗，但生存期未见明显变化。结论：GIST 消化道症状无特异性，术前确诊率低。肿瘤的大小是判断良恶性 GIST 的重要指标。目前间质瘤的治疗仍以手术切除为主，对复发或远处转移者应积极再次手术，可延长生存期。关键词 胃肠道；间质瘤；临床分析2The Diagnosis and Treatment on Gastrointestinal Stromal TumorsA

9、肌源性或神经源性肿瘤。近年的研究表明，此类肿瘤并不同于真正的平滑肌瘤和神经源性肿瘤。我院自 1994 年 10 月4至 2004 年 10 月收治了 27 例，现报告如下。1资料与方法11一般资料本组 27 例中，男 18 例，女 9 例；年龄 32 岁65 岁，平均48.6 岁；发病至就诊时间为 9 d6 a，平均 8 个月。1.2 肿瘤发生部位、大小及性质 本组 27 例中，胃间质瘤 13 例(48.1)，空肠间质瘤 7 例(25.9) ，回肠间质瘤 5 例(18.5 )，十二指肠、结肠间质瘤各 1 例（各 3.7） ；瘤体直径 5 cm 17 例，瘤体直径 5 cm 10 例。经病理证实
10、为恶性间质瘤 6 例(22.2)，瘤体直径(7815)cm；良性间质瘤 21 例 (77.8)，瘤体直径(3912)cm，瘤体差异有显著意义(P5 cm2 。本组结果显示，恶性者瘤体大小为(7815)cm。良性者瘤体大小为(3 912)cm ，两者差异有显著意义(P005) 。33GIST 临床表现GIST 可发生于各年龄段，绝大多数病例发生在成年人，本组平均年龄 48.6 岁。GIST 临床表现为非特异性症状。常见有消化道出血、腹痛、腹胀、腹部肿块、肠梗阻、肠套叠、慢性贫血等。本组腹痛腹胀 70.4，消化道出血占 48.1。有些病例以转移灶为首8发症状，本组中有 1 例以肝肿块来就诊。临床症
11、状轻重与良恶之间无明显差别。34GIST 的术前诊断GIST 的诊断比较困难，术前确诊率低，内窥镜和消化道造影检查是术前诊断的主要手段。但由于 GIST 肿块位于黏膜下。内镜检查只可显示黏膜下肿物或表面黏膜是否完整或有溃疡等，所以内镜下不易取到肿瘤组织。本组 13 例胃间质瘤，内镜病理诊断率为0。文献报道采用超声内镜下穿刺活检，选择性动脉造影术，可明显提高确诊率4,5 。钡餐 X 线造影可显示充盈缺损或黏膜光滑或龛影，B 超和 CT 检查可显示肿物，低位直肠间质瘤可直接切除组织活检。若联合采用内镜、消化道造影、B 超和(或)CT 检查，结合临床表现可提高术前确诊率。35GIST 病理学检查近年
12、来发现 GIST 是起源于胃肠壁肌层中的卡哈尔间质细胞的肿瘤。CKit 是 GIST 特异性和敏感性标志物。有作者认为从总体上说 GIST 是一种具有恶性倾向的肿瘤。其病理诊断与生物学行为不一致。本病组织学类型虽为良性，但临床表现为复发转移3，6 。本组资料显示，随访的 18 例“良性”患者行根治术后，92 例分别在 6 个月和 1 a 后复发，6 例“恶性”者 1 例多器官转移，2 例死亡，3 例局部复发。所以，在判断 GIST 良恶性时，不能只依靠病理学诊断，而要结合临床表现。一般肿瘤体积越大，恶性的可能性就越高。36GIST 的治疗目前，GIST 的治疗以外科手术切除为主要手段，且以局部切除为主，切缘距肿瘤 2 cm3 cm 即可，一般不需行区域淋巴结清扫。本组 29 例标本检查均未发现
