1、摘要手术患者术中压疮危险评估及护理防护摘要目的: 为了有效地预防和减少术中压疮的发生，提高术中护理质量，故本研究对手术病人进行压疮风险评估，针对评估出的结果采取护理措施来进行观察。方法: 应用手术患者压疮风险评估表抽取2016年6月-2017年6月西电集团医院手术室手术病人3685例从年龄、体重指数、危险部位皮肤情况、预计术中施加的外力、体温、活动情况、合并症、循环情况、预计手术时间等方面进行全面评估，并进行评分，根据分值的高低来制订既能满足手术需求又能杜绝术中压疮发生的护理措施方案。记录并观察术后受压部位皮肤颜色情况。结果：经过术前对病人发生压疮的危险因素进行手术压疮评分，评估分12分为低度
2、风险，其中低度风险1020例，占手术总数的27.68%，13-21分为中度风险2130例，占手术总数的57.80%，22分为高度风险535例，占手术总数的14.51%，对评估出有压疮风险的手术患者采取有效的护理防护，如：压疮帖、体位软垫、保温毯等，患者离开手术室前检查其皮肤情况，无皮肤压疮的发生。结论：手术中压疮的发生率是评价手术室护理质量的重要指标，通过使用手术患者压疮风险评估表进行有效正确的评分，采取安全有效的护理防护措施，可以防止术中压疮的发生率。关 键 词：术中压疮；风险评估；护理防护论文类型：应用研究 In this paperPurpose: in order to effecti

11、是因身体局部短时间内过度受压引起血液循环障碍，造成皮肤以及皮下组织坏死，其发生时以压力为主、多种危险因素综合作用的结果，由于手术中存在一些非人为控制的因素，致使手术患者成为院内压疮发生的高危人群2，1998年，荷兰的教学院和综合性医院中的压疮发病率分别再13.2%和23.3%,其中各有39.1%和21.7%压疮患者经历手术治疗。术中因足跟部和骶尾部发生压疮的外科手术病人占52.9%，骶尾部占15.7%3，李华萍4报道老年患者受压部位损伤发生率55.83%；在有效防护措施的前提下，肝移植手术压疮发生率为37.5%，发生在头枕部及脚后跟5；而俯卧位脊柱手术患者急性压疮发生率为0.7%6。在侧卧位开
12、胸手术中，压疮发生率为20.96%，最常见部位为腋下侧胸部，占50%7。因此，防范与减少压疮的发生也是手术患者安全管理目标之一，同时也是评价手术室护理工作的重要指标，近些年，对于术中压疮的风险管理工作也得到广泛关注与重视，应用有效的压疮危险因素评估表（risk assessment scale,RAS）对患者的状况进行客观评估是压疮预防最关键性的一步8。最常用的压疮危险因素评估表有Braden量表，Waterlow量表，Norton量表，其中Braden量表是目前临床采用较为广泛的压疮危险因素评估量表，在评估压疮风险等级方面具有明显优势，但由于部分项目与手术室护理工作不符，并不适用于手术室压疮风险评估，故我院自行设计了手术患者压疮风险评估表旨在更准确的评估手术患者压疮风险，有效的预防和降低了术中压疮的发生，同时采取有效的护理防护，进而提高手术护理的质量。1.2 研究目的及目标 目的：为
