12、万忠和 罗福漳 关紫云 刘健玲 陈 焰 赵学峰 陆丽明 陈振华 谢春生 高丽梅 区洁芬李伟煊 林坚弟 林肇得 吴茜英 徐雁贤 黄碧坚 徐坤仪 陈敏涛 南海医院医院感染预防与控制专家组的职责1、对医院感染预防与控制工作进行业务指导；2、对医院感染发生状况及危险因素进行调查、分析；3、对医院感染暴发事件进行报告和调查分析，提出控制措施并协调、组织有关部门进行处理。本院医院感染暴发报告流程（一）当同一科室短时间内发生5例以上疑似医院感染暴发或3例以上医院感染暴发，任何医师、护士都有责任立即向本科室医院感染管理小组组长汇报，并电话通知医院感染管理科和医务科等相关科室。同时填写医院感染病例报告卡，送医院
13、感染管理科。医院感染管理科应立即到达现场进行调查处理，采取有效措施，控制医院感染的暴发。并于6小时内报告主管院长和通报相关部门。（二）经调查证实为医院感染流行、暴发时，应当于12小时内向南海区卫生局报告，并同时向南海区疾控中心报告。（三）当科室短时间内发现5例以上的医院感染暴发；由于医院感染暴发直接导致患者死亡；由于医院感染暴发导致3人以上人身损害后果；发生特殊病原体或者新发病原体的医院感染；可能造成重大公共影响或者严重后果的医院感染，应立即向科主任汇报，并电话通知医院感染管理科和医务科等相关科室。同时填写医院感染病例报告卡，送医院感染管理科。医院感染管理科应立即到达现场进行调查处理，于2小时
14、内报告主管院长和通报相关部门，并组织专家进行流行病学调查处理。（四）经调查证实出现医院感染流行时，医院于6小时内向南海区卫生局报告，并同时向南海区疾控中心报告。本院医院感染暴发处理流程（一）迅速组织医务科、感染控制管理科开展医院感染控制措施及流行病学调查工作，并在人力、物力、财力等方面予以保证，使感染控制有序、高效率的开展，将受感染人群缩小到最低范围。（二）科定及时查找原因，协助调查和落实感染控制措施。（三）检验科负责各种病原学的检测。（四）感染控制管理科进行流行病调查1、证实为医院感染暴发，对怀疑患有同类感染病例进行确诊，计算其罹患率。2、制定和组织落实有效的控制措施。隔离病人，积极治疗。对
15、科室的用物、场地进行正确的消毒灭菌处理。必要时，暂时关闭病房，停止接受新病人。3、积极查找感染源，对感染病人，接触者、可疑传染源、环境、物品、医务人员等进行病原学检测。4、查找引起感染的因素，对感染病人及周围人群做详细的流行病学调查，认真填写调查表格。5、分析调查资料，对病例在科室的分布、人群分布、时间分布进行比较，分析原因，推测可能的感染源、感染途径和易感因素，结合实验室结果和所采取控制感染措施的效果综合作出正确的判断。6、根据感染暴发或流行的调查和控制情况，写总结报告，制定防范措施。 医务部 2010-5-6附医院感染暴发报告及处置管理规范医院感染暴发报告及处置管理规范第一章总则第一条为规范医院感染暴发报告的管理，提高医院感染暴发处置能力，最大限度地降低医院感染对患者造成的危害，保障医疗安全，根据医院感染管理办法，制定本规范。第二条 本规范适用于各级各类医院，其他医疗机构发生的医源性感染暴发的报告及处置工作依照本规范管理。第三条医院感染暴发报告范围，包括疑似医院感染暴发和医院感染暴发。第四条医院感染暴发报告管理遵循属地管理、分级报告的原则。第五条卫生部和国家中医药管理局负责全国医院感染暴发报告及处置的管理工作。 县级及以上地方卫生、中医药行政部门负责本辖区内的医院感染暴发报告及处置的管理工作。第二章组织管理
